When I arrived at the location there was a backhoe sitting below the dike. Nobody was at the location working on the cleanup today. They removed dirt below the dike and removed dirt and grass in the area affected by the scald to the dead evergreen tree. Inspection of the well found fluid to be seeping out from the threads on the tubing head. There was also gas leaking out around the bull plug on the 8.625 inch diameter casing head. I notified Sam Johnson of my findings at the well. |

Inspection of this well brought me to the tank battery first. The identification at the tank battery was not correct or filled out properly. The tank battery was comprised of a 210 barrel steel tank with no plugs in the top of the tank, meter and a separator. The dike drain was open at the end and the cap that was on the end was unscrewed. There was a very significant and large scald area out side the dike. That using my gps I concluded was about 75 feet wide and traveled 250 feet downhill. There were at least 4 cedar trees in the affected area 3 of these trees that were closer to the source were heavily distressed. The production tubing in the well was shut in and disconnected. The 4 1/2 inch diameter pipe was leaking around the top of the tubing head no gas was detected and it was just clear fluid. A field test with Quantabs maxed out the low strips indicating at least 600 ppm chlorides. The 8 inch diameter casing was leaking below the casing head. These two leaks were causing pollution and contamination around the well head. Further investigation will be needed to deduce why the 4 1/2 inch diameter and 8 inch diameter were full of brine causing the casing heads to leak. When we returned to the truck Sam Johnson, Productin Manager for Condevco Washington Operations, pulled up. He addmitted that the tank over ran back in June or July. He said that he tried to call Andy Thomas but the number was wrong. He said that they seeded and mulched the area then. All contaminated soil will be removed and taken to an epa approved landfill. Noncontaminated soil will be brought in to replace the contaminated soil removed. All affected areas will be seeded and mulched to allow vegetaion to reestablish to prevent erosion. The dike drain shall be made to prevent the escape of fluids. The tank battery shall be properly identified with the Company name and telephone as well as the other required inf |

The wellhead is equipped and repaired in a way that there is no fluid actively leaking from the well. There is proper identification on the wellhead and the tank battery. The tank battery is equipped with a 210 barrel steel tank and a vertical separator. There is a proper earthen dike built around the tank. There are no signs of the original pollution and contamination, the well is properly identified, and the leak has been repaired on the well. This well is now in compliance. |
