0450010770 2014-12-31 Production Wells No Violations I was on site to conduct a status check. The wellhead and tank battery have separate access entrances. . The tank battery access road entry is on the south side of Campbell Road (CR 840) approximately 0.03 miles east of Mardis Road (CR 826). The tank battery access GPS coordinates are 40.75348, -80.961308 +/-9ft. Identification was posted on the tank battery. The tank battery consists of one 210 bbl production tank and one horizontal separator. The tank valves are plugged. There is a meter box with a current chart showing production. There is no dike present. The wellhead is equipped as a plunger lift. The access road and site are stable. The operator will be contacted to address the dike issues.
0545318839 2017-03-29 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1360. Lease access to the well was stable. Fallen trees blocked drivable access to the well head. The well head location was within woodlands and was stable. No identification was present at the well head. The 8.625 well head was below grade. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No audible or visual leaks were detected. The tank battery consisted of one (1) horizontal separator and one (1) 210 bbl production tank. The battery was located less than 100 feet from Campbell Road and no earthen dike was present around the tank separator. The separator was in good repair. No audible or visual leaks were observed. A current 16 day production chart was in the meter house. Correct and legible identification was present on the production tank. The tank body was in good repair with no holes or leaks. The load line valve was closed and plugged. The drain line valve was closed, locked and plugged. The hatch lid was closed. The location of the tank battery upgrade of Campbell and Mardis Roads of Hanover Township and lack of an earthen dike poses a risk to life, health and property by not being able to prevent the escape of substances resulting, obtained, or produced in connection with the operation of well #2-1360 onto a public road (Campbell or Mardis Roads) or other areas that could cause a safety hazard in accordance with 1501-9-9-05 (A)(7) and 1501-9-9-05 (A)(9). I will contact David Beck regarding the posting of identification at the well head and construction of a dike around the production tank.
