0729864468 2012-10-09 Drill / Deepen / Reopen No Violations I went to the site to check on the progress of completion of the surface casing installation. Nomac 310 had spudded the well on 10/07/2012, and TDd the surface hole at 591 KB on 10/07/2012. The rig had installed the 13.375 OD casing to the bottom of the hole and attempted to circulate. The casing pressured up, as it appeared to be packed off. The rig began to remove the casing, and got hung up. The casing parted and part of the casing string fell down hole. The rig had to fish it out. At the time of my site visit, there was still one joint of casing in the hole. Fishing would continue until the last joint was removed. A new string of surface casing would then be installed.

-Tex Pumping was the cementing contractor. The cement job was done in stages. Following a spacer designed to recover the oil-based mud used to drill the hole and condition the hole, a scavenger cement followed by a tail cement were pumped. The scavenger cement was mudscaled at 13.0 pounds per gallon (ppg). O-Tex pumped 229 bbls of scavenger cement at 13.0 ppg. O-Tex then pumped 393 bbls of tail cement at 15.3 ppg. The plug was dropped, and displaced with 283 bbls of water. When the displacement volume reached 195 bbls, circulation was lost. According to a representative of O-

The cement was mudscaled at 15.6 pounds per gallon (ppg), and O-Tex and pumped 106 bbls of cement slurry at 15.6 ppg. Cement returns to the surface were observed. An estimated 8 bbls of cement returned to the surface. The plug was displaced with 96 bbls of water. The plug bumped the float collar at 11:23am. Pressure held. The float held when the pressure was released, and 0.5 bbls of water bled back.

-Tex Pumping was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement was mudscaled at 15.8 lbs per gallon (ppg), and then adjusted to 15.6 ppg. O-Tex pumped 117.5 bbls of cement slurry at 15.6 ppg. Cement returns to surface were observed. An estimated 23 bbls of cement returned to surface. The plug was displaced with 85 bbls of water. The plug bumped the float collar at 4:37pm. O-Tex pressured up on the casing, and the pressure held. When the pressure was released, approximately 0.5 bbl of water flowed back, and the float held.

1409837557 2014-06-17 Preliminary Restoration No Violations I stopped by the site to check on the status of well completion and restoration. The well has been hydraulically fractured. There is two 400 barrel tanks and one GPU production unit. The well is equipped with a Christmas tree. Values are locked out. Tanks or the wellhead do note have ID signs posted. I walked the entire site, and found the location to be in good condition. All erosion and sedimentation controls, and drainage controls appear to be functioning. Ground cover is well established along the access road and around the drilling pad. No areas of erosion are present. Preliminary restoration approval will not be finalized until well is in production and ID sigs are posted.

Nomac 310 was onsite, and rigging up. I reviewed the new well construction regulations with Mr. Kirkpatrick, and provided him with a copy. Drilling superintendent John Snook requested a copy as well. The rig anticipated spudding on Saturday 10/06/2012.
