0003261051 | 2013-11-29 | Drill / Deepen / Reopen | No Violations | I was at the site to check the progress on the conductor drilling operation. Precision Rathole was on location and in the process of drilling the conductor hole for the Pitts 22-14-4 #5. The bit was stuck down due to a mechanical failure, and the rig crew was working to free the bit. A second rig was in the process of drilling the Pitts 22-14- |

-based mud used to drill and condition the hole, a lead cement followed by a tail cement were pumped. The Mudpush spacer was mudscaled, and pumped at 12.5 pounds per gallon (ppg). Schlumberger then pumped 255.8 bbls of lead cement at 13.5 ppg, and 316.9 bbls of tail cement at 15.0 ppg. The plug was dropped and displaced with 308 bbls of water as measured by pump truck tanks. The plug bumped the float collar at 2:08pm on 03/15/2014. The plug landing pressure was 3255 psi, and it was raised to 3917 psi, and held for 5 minutes. Pressure held. The float held when the pressure was released, and 4.0 bbls of water bled back.

0573245417 | 2016-03-29 | Production Wells | No Violations | I was on location for a status check. My inspection found the access road gate locked and the
access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #10H well is in production. The well is equipped with a christmas tree valve assembly. There is a steel dike around the tank battery. The equipment on the pad consists of the following: 6 - Storage tanks 400 bbl 1 - Vapor recovery unit (VRU) 6 - Gas processing units (GPU) 1 - Vertical separator 6 - Sand separator 1 - Thermal electric generator 6 - Producing wells Site fenced and man doors locked |

-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Along the interior of the chain link fence there are two site drains installed. There is an earthen berm, with well-established ground cover. The soil stockpile is well stabilized with vegetation. Crops, woodlands and/or underbrush are growing normally outside the area of disturbance. Drainage and erosion controls outside the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary
