0408287242 2016-10-27 Plug / Plug Back No Violations Status check inspection on the current well plugging process with Harmons Well Service rig on the hole. The rig ran in the hole with a casing ripper and set down at 967 inside the 5.187 casing, they then began attempting to rip the casing at 525 where they had telegraph. After 5 hours on location they were unsuccessful in getting the casing ripped. The contractor will continue their efforts to rip the casing and if unsuccessful will bring in a wireline truck and shoot the casing off with a electric detonated explosive cutter.
1074696931 2016-08-15 Plug / Plug Back No Violations On 08/12/2016 I received a phone call from a service contractor identified as Troy Harmon who has been contracted to plug the said well; Mr. Harmon indicate he would be moving in and wanted conformation thy had the correctly identified the well when applying for the plugging permit. Mr. Harmon also requested a recommended method of plugging and was informed to remove the tubing and rods and draw the well bore fluid level down to the shot hole; and then properly emplace the clay as the bottom plug according to 1501:9-11-09, once the bottom plug was verified as properly set then the removal of the 5.187 string (already in place and set as a liner shall begin) Mr. Harmon was informed that the well bore was to remain bailed down and the well bore filled with clay while pulling the 5.187 casing string. Mr. Harmon indicated that he wanted to run a 1 tubing string as a fluid displacement string and not bail the fluid level down. I informed my direct Supervisor Brent Bear that the contractor was not following my directed plugging recommendation.
1471363364 2016-08-16 Plug / Plug Back No Violations On site status check inspection found Harmons well service rigged up on a well with tubing and rods pulled out of the well and the company prepared to start plugging the well with clay. Upon review of the well location it was discovered that the Harmons were preparing to plug the wrong well on the lease. The well they were rigged up on was 3411120714-0000: confirmed by the coordinates of EPE 20-39.72383 X 80.96986.
1477061488 2016-10-21 Plug / Plug Back No Violations On site inspection to witness the clay plugging process- Harmons Well Service started plugging with clay on 10/20/2016 (See inspection report). On 10/21/16 I witnessed Harmons Well Service run in the hole and tag bottom with a dump bailer at 1,648 measured depth by wireline. They rigged up to begin plugging and had no water in the tank to mix clay and were not prepared to pump water up the hill to the tank. Instructions were left to plug with clay solid up to at least 1,300 before attempting to begin recovering the 5.187 casing liner.
1477348587 2016-10-24 Plug / Plug Back No Violations On site inspection to witness the clay plugging process with Harmons Well Service rig on the hole. The well bore total depth was tagged at 1,648 and filled up with clay to 1,451 with the well bore consuming at total of approximately 8 tons of clay in the 3 days worked to date. The service rig crews continue to grout with clay while confirming total depth.
1477924854 2016-10-31 Plug / Plug Back No Violations On tis day I witnessed Harmons Well Service rig on the hole pulling the 5.187 outside diameter production string which was ripped at 321 on 10/29/2016. The rig is pulling 2 joints at a time and topping the 5.187 off with clay inside as the pull the casing allowing the 5.187 to serve as a dump bailer filling the open hole as it is pulled. The well was filled to surface on this day completing the plugging process.
1529426044 2018-06-19 Final Restoration No Violations Status check inspection found the well site restoration meets ORC 1509.072 with vegetation established and erosion control in place. All equipment has been removed. There is still one well on the property that was permitted to plug.
