0019830824 2015-02-17 Administrative Inspection No Violations Spoke over the phone with Mr. John Thompson of Double D O&G at 10:38 am and he stated that Patchwork Services is preparing to rig up over this well to run 4.5 casing into the hole and will set an open hole packer below the 6.625 surface casing. Patchwork Services may have to bring in a power swivel to re-achieve TD as Mr. Thompson suspects the well is bridged off in the open hole section. After TD is achieved, 2 tubing will be run in and the well will be put back into production.
0199361990 2014-07-31 Production Wells Violations Noted Performed an inspection on this well in response to a landowner complaint. Upon inspection, it was observed that the well is not capable of production. The well consists of a pumping unit over 8.625 surface casing and a gas escape packer with 2 tubing and rods. No lines are hooked up to the well head, and there is no electric service to the motor for the pumping unit. There is a 100 bbl production tank located approximately 50 away from the well with 4 of fluid inside it. The tank is hooked up to the other well on this property. There is no containment dike around the production tank. A containment dike built to SPCC standards needs to be constructed around the tank. No identification information is posted at the well or the tank. Production records for 2013 show that 18 bbls of oil and 0 Mcf of gas were produced from this well. No oil production has been reported from this well in past years, gas only. This well shall be plugged and abandoned. The operator will have 180 days upon receipt of this notice to reach compliance.
0324095276 2015-05-26 Production Wells No Violations Met with Mr. John Thompson of Double D O&G to discuss location and setup of new tank battery to gain compliance with SPCC and spacing requirements. It was decided that the existing 80 bbl tank will be moved and used as the brine storage tank and a new 100 bbl production tank will be set alongside it. A new dike will be constructed to meet SPCC guidelines. Mr. Thompson stated that he plans to have the project completed by the end of June.
0468983462 2015-03-02 Production Wells No Violations On location to check status of service work on this well. Patchwork Well Services is on location and rigged up over the well. I spoke with Mr. Tom Dille of Patchwork and he stated that they are preparing to run 4.5 casing with a Parmaco set-down packer to the bottom of the 6.625 casing, which is 1400 deep. The packer will have a 149 tail of 4.5 casing below it to attempt to hold back an area which has created a partial bridge in the well bore. After running the packer, Patchworks crew will attempt to sand pump through the partially bridged area in the well bore to re-achieve TD of 1800.

I spoke to Mr. John Thompson of Double D Gas & Oil in person and inquired about the status of this well. Mr. Thompson stated that they are in the process of acquiring 4.5 casing to run into the hole and put the well back into production.

1289931455 2015-04-01 Production Wells No Violations Performed a compliance followup on this well as training for newly hired Inspector Matt Fulk. Upon inspection, it was observed that the brine disposal line has been routed into a new plastic brine storage tank. The production tank is scheduled to be moved and diked within the next month.
1450809860 2015-12-21 Production Wells No Violations Upon inspection, it was observed that the well is in production. The well was pumping upon inspection and no leaks were observed. The production and brine tanks have yet to be re-located and diked. The production tank is nearly full, Mr. John Thompson stated he will have the tank moved after the next load is taken out of it.

1837623811 2015-03-24 Administrative Inspection No Violations Received a call from Mr. John Thompson of Double D O&G about the progress of the rework on this well. The well itself is complete and capable of production. The tank still needs relocated to an area where an SPCC compliant dike can be built and a secondary brine storage tank can be accomodated. Mr. Thompson estimates that the location will be in compliance within a month.
1912746278 2015-03-23 Production Wells No Violations Upon inspection, it was observed that this well is now capable of production and is producing. There is now rods and tubing in the well and the pumping unit now has a gasoline motor in place. The polish rod is shiny, indicating recent pumping. The valves on the 2 tubing and 4.5 casing are in the open position. There is a 1 steel line connected to the 4.5 casing which runs toward the meter for this lease. There is a 1.25 black plastic line running from the 2 tubing to the small knockout tank on top of the production tank. There is still a brine disposal line coming from the knockout tank that needs to be eliminated immediately. The production tank also needs an SPCC compliant dike built around it and ID needs posted for both wells using this tank. I left Mr. John Thompson a message about these issues and am waiting to hear back.
