The tanks must have a spark arrestor, low-pressure relief valve and carbon filter. In addition, there was signage at the well, but reflected the previous owner. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance. |

The 8.625-inch surface casing is exposed but not gauged as required under specific permit conditions. The owner must remedy the following by 04/04/2016: secure the load-line valves with locks.
