On this day I was on site from 12:00 am to 13:00 pm standing by and monitoring the gas levels along with OBriens Safety Services . Also the initial incident objective meeting took place @ 7:00 am at the Sardis Fire Department with Wild Well Control Company along with Triad Hunter personnel present with the daily operational objectives set and a initial plan to access the well pad and evaluate the gas levels and direction of travel along with the initial well head condition evaluation conducted by Wild Well Control and Triad Hunter personnel. Once the initial evaluation was conducted all personnel then reconvened at the Sardis Fire Department and at that time the operational work objectives for the site pad were set along with the site safety work zone boundaries. Monroe County EMA and the Ohio EMA Management is also on site at the now designated Incident Command center at the Sardis Fire Department.
0652070995 2015-01-21 Completion Testing No Violations Follow up inspection to a report received by Division Engineer Tech Andy Thomas on 01/20/15 @ 5:14 pm of possible well product laying inside of 5 x 5 pits dug on location to erect the production flow lines above ground related to the said wells on location. On this day upon my inspection it was observed that several pits have been dug with 2 flow lines present above ground and fluid laying in the pits. The fluid was site tested for chlorides with High and low level Quantab test strips indicating chloride levels elevating higher closer to the said well with High level test strip showing 4,121 (PPM) parts per million and exceeding the reading capabilities of low level strip tests. The pit tested at the farthest point from the well showed 696 PPM on the high level test and 148 PPM on the low level tests strip. Contact was made with Sam Miracle with Triad at 10:34 am on this day and they currently have a truck in route to recover the product and will continue to recover all surface water created on the well pad until the chloride levels are acceptable. Mr. Miracle also indicated the well pad repairs in place should be done this week and the work over rig will be moving in sometime during the week of 01/26/15 to drill out the isolation plugs on the said well. The well will then be put into production.

0890280706 2016-02-16 Preliminary Restoration No Violations Status check inspection found the well head equipped with (2) 5.185 master control primary gate valves assembled on the B-section well head along with (2) tubing head master valves and a primary control hydraulic emergency shut down in line valve on the tubing head stack located between the 2 tubing manual valves. There is a sand trap separator hooked into the production outlet flow line near the well head supported by a 100 barrel production tank located on location near the well head. The well does have production tubing in it and the well is currently in production with a total of 10- 400 barrel production tanks located on a production pad adjacent to the well pad with 7 gas processing units along with a gas compressor also located on the adjacent production pad. The well was put back on line after being turned over to the companys production department on and after 12/17/2015. Addressing the preliminary restoration- upon inspection of the well pad it was observed that the primary perimeter boundary affected by the well pad construction is secure and has no stability or erosion issues with vegetation established on the well ad perimeter. The well pad itself has earthen material that was stockpiled after a well control incident during December of 2014, According to Sam Miracle with Triads Engineering Department the material is slated to be addressed once the weather breaks this spring. There is also a large portable polymer lined water tank on the well pad location that is being emptied of all fluids currently and is to be disassembled and removed from the locating in the future.

Wild Well Control met with company and AOL, EMA, ODNR and the highest level of gas reading was at 3 that was recorded at the rudders house hold. The gas has been fluctuating between 1-3 LEL. EMA has requested a 24 hr. monitoring. The Hot Zone is still on drill pad. The warm Zone is on first stage area to the guard shack. From Guard shack to road is neutral. Wild Well has requested the roads to be blocked during work times. They have deemed that this will be a no fly zone area. They will be opening well up continually to let well breath. The pressure will increase and become louder. WWC will be working on getting flange removed. They will be inspecting the well head during this time. Triad will be getting water rigged up with pumps and getting containment set up. There will be radio communication from guard shack to Command Center.

No plug or shoe ran and casing is to be pressure tested.

1382489396 2013-10-22 Drill / Deepen / Reopen No Violations Inspection and status check on the current drilling progress found the company pulling out of the hole with the drill string after running in and drilling out the 9.625 casing DV tool and tagging the cement top at 9600 with the top of the float collar at 9660. It was discovered during the cementing process that the DV tool was not closing and the well was shut in allowing the cement to set up. The company ran a Bond Log which showed good cement presence and shall now start to prep their casing string to set a casing patch over the open ports of the DV tool.

The proposed total drilling depth shall be 16,200.

1390459410 2014-01-22 Completion Testing No Violations Status check inspection on the completion process found that during the process of drilling out the completion plugs the coil tubing failed and broke in half between the well head control device and the coil tubing reel, the well head blow out preventer was closed and the tubing sheared off, the well was then killed by the means of a top hole kill from surface with prepared mud, the company now has a snubbing unit on location and will recover the 12,677of 2.375 coil tubing left in the well bore.

4:36 PM I received notification from Rob Beardmore with Triad of the well control situation and that they had dispatched Wild Well Control to respond. Deputy Chief Dave Claus was immediately notified.


11:30 PM-- Wild Well Control- Sardis Fire Department members and I present. With the following objectives set to be addressed the next day starting @ 7:30 AM- Water supply is to be re-established to the well pad by 3rd party contractors. Wild Well Control will receive their operational equipment by truck shipment and evaluate the natural gas being released from the well with the objective of setting up water monitor nozzles to diminish the gas being omitted into the atmosphere. Earth moving equipment will also be sent to the well pad to

1418670641 2014-12-15 Completion Testing No Violations Triad Stalder well head gas leak Incident report. Incident objectives for the day are digging pit and establishing a flow ditch between the well and pit, the ditch and pit is to be lined with a plastic liner. The main water supply to the well pad from the Ohio River should be re-established today. Once the water supply and flow ditch and pit are in place the concrete pad will be removed around the well head to allow the crew from Wild Well Control to gain access and a safe working area around the well head. The well pad berm area has been evaluated and the berm grade has been built up to a height to not allow any fluid to leave the pad once water movement operations begin, with all well site drains in the closed position. The well continues to discharge gas only with no well bore fluid present. Ohio EMA continues to support the local EMA at the Incident Command Center located at the Sardis Fire Department. I will remain on site until 5:00 pm and Inspector Dave Ball will then be representing the Division until 1:00 am on the 16th, Brent Bear is working with Dan Goins on establishing a relief crew and schedule. I was on site from 9:00am to 5:00 pm.
1418765022 2014-12-15 Completion Testing No Violations On site to monitor well pad conditions due to recent uncontrolled gas flow due to downhole bridge plugs released for some unknown reason and when owner attempted to loosen night cap to place production tree on they encountered uncontrollable gas flow.

1418835409 2014-12-16 Completion Testing No Violations Report to document the current well head gas leak status. On this day I arrived at the Incident Command Center located at the Sardis Fire Department at 6:45 AM to take part in the 7:00 AM daily work schedule briefing. On site was the Crew from Wild Well Control- State and Monroe County EMA staff - Ohio EPA -Triad Hunter personnel -along with various 3rd party contractors and Sardis Fire Department members. With the following work agenda for the day outlined in discussion. An attempt will be made to place the well head night cap back into position on the well head lower (B) section. I then traveled to the well site and remained on standby until 3:15 pm and at that time I was relieved by inspector Jake Glascock. At the time of my departure a crane was being set up on location with the intention of placing a weighted object on the well head to force the night cap back into position.
1418915173 2014-12-18 Completion Testing No Violations Status report of the current well head gas leak situation. On this day at 7:00 am I arrived at the Incident Command Center at the Sardis Fire Department with a daily pre-work briefing setting the days work objectives. I then traveled to the well site and from 7:30 am to 10:00 am observed excavation taking place around the well head to allow access to the well head from the side and below grade level. Wild Well Control is on location coordinating the efforts and has received by truck shipment from their Texas yard the sand-water pressure jet cutter to be used to cut the damaged well head off. A 3rd party contractor identified as McCutcheon Enterprises , Inc continues to provide remote air quality monitoring by the means of 5 remote transmitter devices placed in the outlining area of the well pad. Work will continue on this day to excavate the area around the well head with the pre-buried well production flowlines that have been unearthed scheduled to be water jet cut and removed this afternoon.
1418980774 2014-12-17 Completion Testing No Violations Inspection revealed that WWC was prepping site to cut wellhead off and replace it with a new one and AOS was patrolling area monitoring for any high levels of gas.

Note: Status of well still remains the same at this time it is still venting dry gas only at this time and Wild Well Control stated that they should be ready to cap well by the end of the week.

1419014174 2014-12-19 Completion Testing No Violations On site inspection and stand by on this day from 6:30 am until 17:00 pm hours to witness the current well head gas leak repair efforts. At 6:30 am I checked in at the Incident Command Center located at the Sardis Fire Department. I made contact with all Incident Coordinators and then proceeded to the well site and positioned myself at the well site guard shack entry point with the following work activity witnessed on this day- Wild Well Control is on location continuing to excavate a fluid recovery pit and prepare their equipment for the well head removal and reinstallation. Universal Well Service has arrived and positioned their equipment on the well pad. The replacement well head has also arrived.
1419071079 2014-12-20 Production Wells No Violations The fluid recovery pit excavation is finished and the pit has been lined. All necessary equipment is onsite to facilitate the casing cut off operations. All LEL levels are reported at 0%. REI representative arrives at command post to make corrections to Monitor 5. It appears the memory bank was full and needs to be reset.
1419120298 2014-12-20 Completion Testing No Violations Arrived on site to witness daily event of WWC trying to get wellhead secured and possibly place new one on.
1419140432 2014-12-21 Production Wells No Violations I am on site tonight at the Sardis Fire Department which is acting as the command center for the well event. I relieved Kip Thompson after the evening update. He advised me that WWC was on site removing more cement around the casing strings so that they could cut away the casing strings down to the 9-5/8 inch diameter casing which they intend to use for well control. The casing should be cut down to where the bottom plate can be placed over the cut section and the new well head placed down on the 9-5/8 casing to establish well control. AOS advised me that after monitoring the area the readings at 12:00 AM were all -0-
1419344175 2014-12-18 Drill / Deepen / Reopen No Violations WWC has finished excavating around the wellhead and have completed construction on the ramp.

They have rigged up the sand cutter and tested all of the necessary equipment in preparation for wellhead repair. On Friday, WWC will spend 3-4 hours lining the containment pit and digging the drainage ditch. The wellhead equipment will arrive on Friday. They want everything lined up perfectly for cutting of wellhead on Saturday. WWC will need a good solid day to cut the wellhead, so they will begin early. Water management is a key issue for eliminating any spark/ignition potential while cutting the wellhead, so there will be a significant amount of water use. WWC will install the wellhead and cap on Sunday assuming there are no issues. AOS continues to monitor for potentially dangerous gas levels. They reported that none were detected at this time (22:00). Arrived at Guard Shack and there has been no change in status of the well.

1419354773 2014-12-23 Drill / Deepen / Reopen No Violations I was onsite incase there were any emergencies through the night. The gas readings were at zero all night. Wild Well Control should finish up the well tomorrow and remove there equipment from location.
1419381747 2014-12-23 Completion Testing No Violations On site inspection from 7:00 am until 5:00 pm to witness the well head replacement efforts. On this day crews from Wild Well Control finished cutting and dressing the 9.625 @ 5.5 casing strings and attached a new well head assembly, once the well head was pressure tested the well head was closed bring the gas leak under control at 3:36 pm. A wireline company will now run in and attempt to tag the top bridge plug previously set in the well bore and then conduct a casing caliper run. Coil tubing will then be rigged up and used to set a 12,500 psi rated bridge plug and spot a 100 cement plug on top of the bridge plug.
1419529188 2014-12-25 Completion Testing No Violations On site inspection from 6:00 am until 1:45 pm to witness the attempts to set a Halliburton type 12,500 psi rated cast iron bridge plug to be deployed from 2.875 coil tubing with Wise Company coil tubing on location and rigged up on the hole. After several unsuccessful attempts to get the bridge plug to insert into the 5.5 well bore through the well control stack due to a dimensional internal diameter offset between the well control stack and the 5.5 production casing it was decided to call for Blue Dot Services slick line truck and attempt to run the plug on their slick line. Due to high wind weather conditions the well isolation efforts were called off and will resume on Friday 12/26/14( The next working day after Christmas). The well head Blow Out Preventer was shut in and the well isolated. Quantab site sampling was conducted showing only high levels in a per miter of approximately 80 around the well head, Triad had a 3rd party contractor-Carper well service on location recovering any accumulated surface water.
1419627504 2014-12-26 Completion Testing No Violations On site inspection @ 8:30 am to witness the attempt to set downhole isolation plugs in the 5.5production casing with the following observed. We were initially unsuccessful in getting the cast iron 12.5 k bridge plugs to go down hole on wire line, as they would not go on coil tubing on 12/24. So the company man began the process of evaluation and elimination through pressure testing, we were able to determine the problem was ice inside the well head. At around 12:00 noon forced air heaters were placed into use on the well head and brine was pressure injected into the well head, using a pressure-
1419709807 2014-12-27 Completion Testing No Violations Wise Coil Tubing unit still trying to get coil down hole too 9040 right above 12K bridge plug so Universal Well Service can pump 100 cement plug on top of bridge plug and they plan on repeating the same procedure tomorrow if all goes well.
1421925654 2014-12-17 Completion Testing No Violations The plan is to remove all non- needed equipment off of the Stalder pad. The Baker pump trucks are on standby for killing purposes. The crane service will be rigging down and moving off location. Wild Well Control is going to build and construct access to the well. Triad will construct flow ditches for residual water and containment.

10:00 am Equipment is being moved off of the pad. At 12:00 noon Wild well control should have an established time limit on the required work. The Ambient air Quality is at a .4ppm LEL. Triad stated that there will be a total of two days of excavation, two days of disassembly and removal and two days of reassembly and install the slip well head. Then a final day of testing. 3:00 pm excavating still remained in operation. All housing trailers, campers, roll-offs, Halliburton equipment, wolf Pac battery supplies have been removed off site to keep gas from laying in low areas. Dave Ball and I switched at this time. There was no change in the ambient air samples.

1423180898 2014-01-30 Completion Testing No Violations Status check inspection on the completion process found Currey well control snubbing unit on location with the company currently conducting their Blow out preventer tests and preparing to run in the hole with 2.875 tubing and a down hole drill out motor and finish the completion process.
1432850019 2013-11-06 Drill / Deepen / Reopen No Violations Inspection and status check of the current drilling progress found the current drilling depth to be 9260 with the company man reporting they have started their deviation off of the cement plug at a 5 degree build rate. The current oil base drilling fluid being used has a weight of 11.3 # and a viscosity of 122 with the borehole azimuth of 195.5 degrees.
1468434077 2014-12-24 Completion Testing No Violations Inspection revealed that coil tubing unit was rigged up and after several attempts to get tools started in hole failed they called wire line truck back to site to attempt to get bridge plug started in hole.
1522127197 2013-09-09 Drill / Deepen / Reopen No Violations I received notification on 09/07 that while drilling the 1st intermediate borehole a hammer bit and sub was lost at 2024 and the hole had collapsed up to 1452 before they could get back to bottom with the open ended drill string. A plug was set on 09/08/13 from 1452 back to 585, which put them back up in their surface casing. The company plans to deviate out of their original borehole and continue drilling the well bore, Baker & Hughes cement services used 2282 sacks of class A cement. Records were received.

1549985966 2019-02-12 Production Wells No Violations On site inspection follow up to a report I received on 02/11/2019 from ODNR Engineer Technician Andy Thomas that the said well was bubbling gas from inside the tin horn cellar. On this day I observed there was fluid standing inside the tin horn cellar to ground level. There was bubbling coming from what appeared to be around the well head area from below the fluid level inside the cellar. Annular pressure monitoring is in place on the well head A & B sections with 2 riser pipes present equipped with valves and gauges. The A section shows 400 pounds pressure and the B section shows 250 pounds pressure. The production tree shows 1,200 pounds pressure on the flow cross before the choke valve. On this day I contacted Bruce Carpenter with Eclipse and discussed the bubbling in the cellar and asked him to have the companys lead point man contact me regarding the bubbling.
1552399037 2019-03-12 Production Wells No Violations Status check inspection found the well equipped with a upper B section with a 2 diameter riser pipe attached to the service ports on the B section. The riser pipe is equipped with a valve and gauge for the purpose of annular pressure monitoring. The B section pressure reads 400 pounds. The 5.5 production casing well head section flow cross is equipped with a gauge that reads 1,459 pounds pressure . The well head is equipped with a production tree and lubricator head with 1,161 pounds pressure on the production tree. The well is flowing through a sand trap separator. On site on this day were company representatives Neil Ingram and George Winland along with a water truck from Carper Well Service. The water truck removed the water from the tin horn cellar so the well head assembly could be evaluated to determine where a pressure leak was coming from.
1605200300 2014-12-21 Drill / Deepen / Reopen No Violations I was onsite to cover and witness the operations to contain this well. The well is still currently venting gas into the air. Wild Well Control has stopped working for the day and will return tomorrow. OAC will be monitoring the air quality tonight . Triad stated the well will be finished tomorrow.
1622995379 2014-12-18 Completion Testing No Violations On the night of my inspection I found that the well was blowing dry gas. AOL was patrolling the area to monitor air quality. There were 5 remote air quality devices in use that were being monitored from the guard shack. The LEL never exceeded 1.3 ppm during my time on post. There was no change in the status of the well during my time on post.
1799945868 2014-12-23 Completion Testing No Violations Stopped at the ICC and signed in and received update on todays activities at the well site and then I proceeded to well site where they were attempting too run wireline tools into well to try and determine where the bridge plugs were at in the wellbore they ran tools to 8200 before lubricator assembly started to leak and decision was made to come out of the hole and rig up coil tubing unit to set new bridge plug.
1945859115 2017-12-07 Preliminary Restoration No Violations Preliminary restoration status check inspection found the area affected by the well pad construction perimeter boundary to be stable with vegetation established. There is a 2nd pad site that us used as a production pad that the lease road to the pad runs through that also has the perimeter established with vegetation, on the second pad is the tank farm that consists of a total of 10 steel design 500 barrel fluid capacity steel tanks situated inside a polymer lined steel containment area. There is also a large gas compressor and 5 gas processing units on the second pad site. Addressing the primary pad site there are 5 wells completed on the pad with all 5 wells found to be in operation on the pad. Proper well identification is posted on each well and at the tank farm battery site. The lease road is of stone construction. Bruce Carpenter with Eclipse Resources recently contacted me and indicated that Eclipse has taken over ownership of the wells and equipment.
