On 10/31/2014 the conductor was cemented with no cement returns to surface. On this day the casing was pressure tested to verify the shoe was properly cemented, with no recorded pressure loss. The annular side of the casing was then grouted through 1 tubing set at 25 with cement verified to surface. Allied Cement services was the cement company using 43 barrels or 256 sacks of class A cement.

Allied Cement services cemented the casing using 328 sacks of class A cement with 100% open hole cement excess ran with no cement returns to surface. Orders were given to the company man on location to re pressure test the casing and confirm a seal at the shoe and run a tubing string in the annular side of the well bore and tag the top of the cement. We will then determine if a grout job or another displacement job needs done. The well pad is constructed atop a old surface mined limestone quarry with approximately 75 of unconsolidated fill material.

Allied cement services grouted the casing through 1 tubing set in the annular side of the casing. 200 sacks of class A -15.6# 1.18 yield 2% calcium .25 pps lcm was used with no cement returned to surface. The company will attempt to grout the casing with a redi mix cement mix on 11/01/14.

The well pad entry point is gated and locked with signage posted. The lease road is graveled along with the well pad, both are in good repair. Addressing the well head - The well is equipped with a B section along with a 9.65 and 5.5 casing well head section and a tubing well head. The tubing well head has 2 manually controlled valves and one emergency shut down valve. Annular pressure monitoring is in place with 500 (psi) pound per square inch indicated between the 9.65 and 13.375 and 1300 psi between the 5.5 and 9.625. Proper well identification is posted at the well head. Also on location is (5) gas processing units one for each well head on location. At the left front corner of the well pad is the production fluid storage area consisting of a total of 8 steel construction tanks all situated inside a polymer lined steel containment dike area. Emergency shut down devices are situated at the well pad entrance and at the tank battery loading site.
