A permit to plug the well has been filed. After my inspection I called Chris Kimble, who stated that he was experiencing a great amount of problems with the well and that the tubing was hung in the hole at approximately 4000 feet. He also stated he would like to come to agreeance on how to properly plug this well with the state. Mark and I stated he would talk to our supervisors to come up with a plan on how to plug this well. |

0660697690 | 2016-05-03 | Production Wells | No Violations | On the day of my inspection, I witnessed this is a swab well, utilizing 4.5 inch diameter production casing with a 4.5 inch to a 2 inch diameter changeover placed on top of it. A do not operate tag was placed on the well head. An odor of gas could be noticed when I was around the well head. Production equipment for this well includes a 100 bbl. production tank, a horizontal separator and a 2 inch diameter meter run; all placed inside an earth dike that measures approximately 40*30 feet at an average height of 4 foot. Proper identification was in place on the tank battery at the time of my inspection. After my inspection I then placed a call to Chris Kimble, who informed me that the well was shut in due to the fact that the production tubing was hung in the hole. Chris stated that the well was on a list of wells to be serviced. I then asked Chris about the gas order coming from the well and he stated that he was unaware of the issue. I will be back at a later date accompanied by inspector to insure that this is not an H2S issue. |

The 2 3/8 inch tubing was run to the depth of 1446 feet. It was then decided that they were in side of the 8 5/8 inch casing and the bottom of the surface casing plug was near where it was to be. The 2 3/8 inch tubing was then pulled up to 430 feet and was then cemented back to surface. The tubing was the pulled and the cement was topped off and it was pressured up to 500 psi and then shut in. |
