Horizontal wirelline perforated the stage in 6 clusters with 40 perforations total. The stage was perforated between the depths of 18,912 and 19,096 and a completion plug will be set at 19,096. The company is monitoring all annuli electronically. All stimulation and support equipment is placed on polymer containment. |

On this day I met on site with Stat Oil employee Brett Pugh who indicated a catch basing drain system situated on the well pad at a coordinates of 39.67850 x 80.89803 had failed and was allowing fluid to by pass the drain and exit on the North East slope. The complaint nature was that soap suds were found in a ditch line directly below the drain outlet however no product was found in the ditch on this day. ODNR Engineer Technician Andy Thomas was notified of the construction defect and was going to address the issue with Stat Oil Construction Supervisor Vinnie Dewiler. The entire well pad perimeter was inspected on this day. |

The rig has drilled a 17.5 diameter well bore to a measured depth of 2,065 and was attempting to run the 13.375 diameter casing while I was on location for 2 hours. They casing tally allowed for 16 of rat hole below the casing and the casing was sitting down 70 from a total casing set depth of 2,049. The company loaded the casing and hole with fresh water however did not install the Franks Casing circulating tool initially and after several hours they decided to lay down the one casing joint and pick up the CRT circulating tool. I left location at 2:00 am and the company man reported that they were able to get the casing installed and past what they believed to be a ledge in the well bore. The job was finished by 7:00 am the next day. |

The well completion operations are to begin in the next 48 hours. |

I found the pad gated with signage at the entrance. There are (3) producing wells, (3) gas processing units, (3) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has annular pressure monitoring devices equipped. Greg Davis, Equinor Representative, was notified to label the gauges and wellheads with proper identification. Mr. Davis also stated that annular pressures are monitored daily. It will be noted that a third-party contractor, FMC Technologies, is onsite with secondary sand traps. |
