0418052296 2018-07-18 Production Wells No Violations On the day of my inspection I found that no work had been done to provide rig access back to the well. This well was required to be plugged by 7/26/18. A plugging permit was issued for this well on 3/23/18. I called Ron Babcock to ask what he was doing. He said that he walked back to the well last month with Phil Moran to plan the plug job. He said that it was still pretty wet getting back to the well and Phil wanted to wait for the ground to dry out some before he moved in. Ron said that he would be on vacation until early August and he would contact Phil to move in when he returned.
0866586419 2018-09-06 Plug / Plug Back No Violations On the day of my inspection Phil Moran had a 16 ton load of clay delivered. Phil tallied 110 feet the 5.5 inch casing that he got out of the well. He ran into issues with moving the service rig and had to hold off on washing any clay down the well today.

I called Phil Moran who said that he came back out yesterday evening to top the well off with the rest of the clay pile and cap it. This well is plugged and abandoned, the compliance notice is resolved.
0967098679 2018-09-04 Plug / Plug Back No Violations On the day of my inspection I found that Phil Moran was rigged up on the well. He called me last week to give notification that he was moving in. He had the tubing pulled out and he had 4 joints of the 5.5 inch casing pulled. Phil said that he had no stretch and he could not get the casing to pull. The records show that there were 5 sacks of cement and 50 sacks of clay used on the surface casing. Phil rigged down and moved his service rig back to the next well. He said that he would not have clay until Thursday.

Phil said that Ron Babcock was just there to spread grass seed on the site. Phil asked for a form 5 landowner waiver for the access road. Phil claimed that he had spoke with Brian Darr, who said that he wanted to leave the access road in place.
1038403096 2018-09-20 Plug / Plug Back No Violations On the day of my inspection Phil Moran washed approximately 3 tons of clay down the well, filling it to surface. The fluid level was high in the well when he started, so he suspected that it would take some time to get an appropriate amount of clay in the well. He said that he would check back tomorrow.
1063973921 2016-01-07 Production Wells No Violations On the day of my inspection I found that the wellhead was equipped tubing and set up for open flow. There was house gas line connected to the casing. I found no production equipment for the well. The production reports are up to date, but they show zero production. I called Ron Babcock and he claimed that the well is used for house gas only, and he has been trying to get with the landowner to sell the well. There were no leaks or spills. I gave him Brian Darrs phone number. I told him that he needs to get something worked out or plug the well.
1383132630 2018-10-03 Plug / Plug Back No Violations On the day of my inspection I found that the clay had fallen approximately 25 feet down hole. I dropped a chunk of wet clay and when it hit I did not hear the splash of fluid, but rather the solid thud of more wet clay. Phil still had a pile of dry clay next to the well. He was going to top it off today once he gets the casing and flow line hauled to the scrap yard.
1516988211 2018-01-26 Production Wells Violations Noted On the day of my inspection I found that the wellhead and old access route was overgrown with thick vegetation The well was equipped with a casing head and tubing. Everything was shut in except one plastic line off of the casing that ran back through the woods toward the Jack Darr #1 well. I found that the line was cut when I walked back to the #1 well. There were no leaks around the wellhead. I found no production equipment, or storage tanks for this well. There was no identification posted at or near the wellhead. I contacted the landowner Brian Darr to ask if this well has ever been, or is currently being used for domestic gas. Brian said that this well has never been used for domestic gas and the only structure on the property is an open wooden storage barn. This well has shown zero (0) barrels oil and zero (0) cubic feet of gas for every year of reported production dating back to 2001. Prior to that there are no records of reported production. The well is not being produced commercially and it is not providing domestic gas. The well owner/operator shall submit a written plugging schedule to Chris Grimm at 5880 Memory Rd, Zanesville, OH 43701 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1536589181 2018-09-10 Plug / Plug Back No Violations On the day of my inspection I found that Phil Moran was not working at plug site. The clay pile was covered with a tarp and it did not appear as though any of the clay had been used. The site was extremely muddy from heavy rains in the area. I called Phil and left a message in regards to when he planned to come back out and finish the job.
1537194380 2018-09-17 Plug / Plug Back No Violations On the day of my inspection I found that Phils 100 barrel water tank had been repaired and he had a load of water delivered. The clay pile was still under a tarp. It was raining and the site was very muddy. Phil said that he was not going to start running clay today. He decided to wait until Wednesday to give the site a couple of days to dry out.
1537547810 2018-09-21 Plug / Plug Back No Violations On the day of my inspection I found that the fluid/clay top was down approximately 50 feet. Phil topped it off with clay again. I questioned that amount of clay that had been used in the well to this point. Phil speculated that the formation was tight and the water had nowhere to go. He said that we will have to leave the well open for days or even weeks to get it to take all of the clay that would typically be used to fill 800 feet of 5 inch diameter hole.
1537972292 2018-09-26 Plug / Plug Back No Violations On the day of my inspection I found that Phil Moran was not working at the plug site. His clay trough was still set up on the well, and more clay had been removed from the clay pile. The fluid level was only down 2 feet. I could not determine the depth of the actual clay column in the well bore. When Phil showed up he said that he got another dozer bucket of clay down the well yesterday, but it is taking it very slow. The bucket holds approximately 2 tons. Phil said that he will have to leave it open and keep putting a little in every day until it is full.
1538075504 2018-09-27 Plug / Plug Back No Violations On the day of my inspection I found that the well was full of clay to within 4 feet of the surface. Phil Moran has been adding clay little by little every day to fill the well to surface. He said that he topped it off again before he left the site yesterday. Phil got an approximate total of 6 tons of clay in the well to fill it to surface.
1874762562 2019-05-06 Final Restoration No Violations On the day of my inspection I found that the well site had bee graded out, but no grass had been planted. There were several piles of brush and trees scattered through the area. I found no materials or debris left at the site. There was no production equipment for this well. I received a form 5 landowner waiver for the final restoration to waive all of the restoration requirements. I called Brian Darr to follow up and he said that they are completely satisfied with how the site was left. Brian said that the County has been dumping all the brush at the site. This site passes final restoration.
