1544110989 2018-12-06 Production Wells No Violations I was contacted by John Loper to ask if I would meet him at his recently purchased property to inspect the domestic well that was allegedly included with the home. On the day of my inspection I found that the ball valve on the 5.5 inch to 2 inch diameter casing swage was closed. We also went to the home and Mr. Loper showed me two separate gas lines coming into the home. One was equipped with a regulator and partially opened ball valve. The other was open flow equipped with a closed ball valve. Mr. Loper said that the home was auctioned as having free gas and the well was the only source of heat for the home. I showed him how to access the well records on the ODNR Oil and Gas website. I also showed him the parcel map, which showed the well was not located on the parcel the he recently purchased. Although the well is currently permitted as an exempt domestic well under Daniel Reeves, it does not meet the statutory requirements of ORC 1509.01(X). The data base showed that the current well owner had $1,000,000 insurance and a $5000 bond. I told Mr. Loper that he would need to have the well transferred into his name as a producing well, and he would also need to carry a bond and insurance. I gave him the contact information for Kathy Smith, with DOGRM Permitting, Surety and Bonding. I also told Mr. Loper that I could not advise him on how to operate the well for domestic gas to the home. I gave him the contact information for a local well tender. Finally, I recommended that he look into an alternate source for heat in the home.

1888599071 2017-03-17 Production Wells No Violations On the day of my inspection I found that the well was equipped for open flow with a 5.5 to 2 inch swage, topped with an open ball valve and into 2 inch steel line. There was a 50 barrel poly tank set next to the wellhead. There was no identification on the tank or wellhead. The 5.5 inch casing was set on a hanger and the surface casing was open to the atmosphere. There was a slight gas odor around the wellhead. I did not find a domestic gas line or regulator connected at the wellhead. There was a black plastic gas line lying on the ground in the direction of Mr. Reeves home, but it was disconnected. The 2 inch steel line also entered the ground in the direction of the home. I called the phone number on record for Mr. Reeves and it was the wrong number. I also called an alternate phone number on record and it was disconnected. I went to the home address on record. No one was home so I left a business card in the front door. The well must be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number, county, and an emergency telephone number. The contact phone number must also be updated with Ohio Division of Oil and Gas Resources Management central office.
