0097583457 2017-01-06 Production Wells No Violations On the day of my inspection at 1142 I received a call from ODNR Inspector Austin Gutridge who stated there was a gas leak at the residence of 11821 Don Wiley Road Caldwell, Ohio 43724 which was reported to One Call. Upon arrival at 1225 at the residence, I met with the land owner Tom Cory who directed me to the leak. I found an area 2 feet by 2 feet of disturbance at Global Positioning System coordinates 39.715966 -81.627397 with natural gas blowing. Readings were taken with ODNR Honeywell PhD6 meter revealing less than 10 LEL gas present and also utilizing ODNR Tango H2S detector revealing no H2S present. I met with Noble County Emergency Management Agency Director Chasity Schmelzenbach and reviewed the call and response. Green Gas South Supervisor Scott Davis and roustabout Rusty Walters were on scene and began shutting in wells that fed into the 4 inch diameter gathering line. A total of five wells feed into the 4 inch diameter line include: 3412116920000 R. Wiley 1, 34121219600000 D. Wiley 1, 34121220400000 Harper 1, 34121218500000 N. Wiley 1 and 34121218800000 N. Wiley 3. The 4 inch diameter gathering line collects gas from all 5 wells and proceeds north towards State Route 78 to a master meter. Arriving at the well I found the production equipment included a 210 barrel transfer tank, a horizontal separator and a 30 barrel horizontal tank. The well was found in heavy vegetation and equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 1.5 inch diameter tubing and a plunger lift system with valves open on the casing and the tubing. Identification was present but illegible and production records were current. Green Gas roustabout Rusty Walters shut-in the well. I then met again with Mr. Davis who stated the wells would remain shut-in until repairs were made next week. Mr. Davis stated that he would notify ODNR Inspector Gutridge upon completion of repairs. 1981679077 2017-01-23 Production Wells No Violations The purpose of my inspection today is to follow up on a gas pipeline leak that was reported and responded to on 1/6/2017. During my inspection I met with the land owner Tom Cory and he directed me to the area of the underground 4 inch diameter pipeline. The pipeline runs through Mr. Corys side yard roughly 40 feet from his house. The portion of the pipeline that was leaking on 1/6/2017 has been successfully repaired. 15 feet away from the repaired portion of the pipeline; an additional leak was found. 6 feet by 2 feet portion of the ground was bubbling in multiple spots from the gas escaping the pipeline. At 11:21 AM I called Mr. Davis with Green Gas to inform him of my findings, he was unaware of the newly found leak, and he stated that he would respond to the leak today. Mr. Davis will report to me his plans to resolve the issue.

2080221020 2017-06-08 Production Wells No Violations The purpose of my inspection is a follow up on a gas line leak. On 5/6/17 I witnessed a 4 inch diameter steel line leaking gas roughly 40 feet beside the residence of Mr. Tom Comey. During my inspection today the 4 inch steel gas line was not leaking. A plastic line had been pulled inside the 4 inch line to seal off the leaking portion. Roughly 400 feet of the 4 inch steel line is now sleeved with a plastic gas line. The proper action has been taken by Green Gas Company to resolve the leak and there were no violations present during my inspection.
2134546090 2017-02-14 Production Wells No Violations The purpose of my inspection was to follow up on a leaking pipeline. During my inspection the previous leak has been fixed. A new leak was detected within 40 feet from a residence. I called Scott Davis with Green Gas Company and he stated he would go out and shut the line off and sometime soon he would replace the portion of the line that is leaking. I was accompanied by ODNR inspector Doug Bennett. Mr. Bennett and I walked a large portion of the pipeline to be sure there are no additional leaks.
