0511296434 2015-09-18 Production Wells No Violations On the day of my inspection I found that the majority of the oily soil had been removed from inside the tank battery. There was still some residual contamination left behind.. I called and left a message with Mr. Barthel to contact me in regards to the clean up.
0814492074 2015-09-22 Production Wells No Violations On the day of my inspection I met at the tank battery with well owner Richard Barthel and Emergency Response Supervisor Trish Cartmell. Mr. Barthel claimed that he hauled two more truck loads of soil from the tank battery. There was still some residual oil in the soil. Mr. Barthel was instructed to remove more soil. He will call me to re-inspect when he is finished removing contaminated soil.
0885942555 2015-09-23 Administrative Inspection No Violations I was contacted by Kathy Smith from DOGRM Central Office in regards to the status of this well. She claimed that her research showed that there was a clerical error made in 1983. The well should have already been listed as an exempt domestic well. She said that she was going to correct the status to show DM, thereby releasing Mr. Barthel from the requirement to report annual production. This violation is resolved.

1282903585 2015-10-02 Production Wells No Violations On the day of my inspection I found that more soil had been removed from inside of the dike. There was still one spot of oil stained soil that needed to be removed. I called Mr. Barthel and he said that he would get it done over the weekend.
1419926459 2015-03-05 Production Wells Violations Noted On the day of my inspection I found that the wellhead was equipped with a pumping unit. The valves were open and there was 20 psi on the tubing. The motor was intact and the polished rod showed signs of use. I found plastic flow line leading from the well to two 100 barrel tanks. This well is in production, yet there has never been any production reported to the State.
1444311935 2015-10-07 Production Wells No Violations On the day of my inspection I met Emergency Response Supervisor Trish Cartmell at the tank battery. We found that all of the contaminated soil had been removed from the dike and the berms had been reinforced with more soil.
1762485734 2015-09-10 Production Wells No Violations On the day of my inspection I met with Mr. Barthel at the well per his request. He claimed that the Township mower knocked the valve off the bottom of his 100 barrel tank allowing four barrels of oil to leak into the dike. He wanted direction for how to proceed with the clean up. Most of the oil had already been removed with a vacuum truck. The remaining oil had been covered with saw dust. No oil had escaped the dike. I instructed him to remove the contaminated soil and he could either have it hauled away, or he could mix it with lime and fertilizer and spread it out on his property to land farm it. While on site I questioned him about the active compliance notice for failure to report annual production. He claimed that the well is used solely for domestic use and it was registered as an exempt domestic well. I informed him that the well was listed as a producing well and explained the steps that he would need to take in order to get the well registered as exempt domestic. I sent an email to Kathy Smith at central office requesting that she send Mr. Barthel a domestic ownership packet.
