0041926409 2019-01-07 Production Wells No Violations Prior to my inspection for Complaint #6049, I called and left a message for the complainant on 12-27-18 after receiving a notification from Sharmon Hughes (Oil & Gas Admin). I did not receive a return call at that time. I called and left a message for complainant on 01-07-2019. The complainant returned my call and advised me he had been previously smelling a strong odor of gas from the tank battery located west of his residence. My inspection found the well equipped with a pumpjack. The well and tank battery have domestic gas connections. The tank battery consists of two steel production tank and a vertical separator. The tank battery is shared with well #25540. The well is legibly identified. There was no odor of gas at the time of my inspection. I spoke with Paul Shaw (Dorfman) he advised the separator dump valve was replaced the previous week. I called and spoke with Mr. Clark about my findings and he advised me he had not smelled the gas odor for a while. I gave Mr. Clark my contact number and advised him to call me anytime he had concerns about the well. I am recommending complaint #6049 be closed. No compliance issues were cited at this time.
0460164528 2007-09-28 Preliminary Restoration No Violations The well is completed as a pumping well. The location is reclaimed. The tank battery (1-shared 140 bbl oil stock tank, 1-shared 100 bbl brine tank) and the wellhead have the correct I.D. signs. The Preliminary Restoration is PASSED.
0639936291 2009-04-23 Production Wells No Violations I met with Steve Clark (330-701-7852) at 4000 Beechwood Ave., Alliance, OH, regarding the new charcoal filter that has been placed on the stock tank at the tank battery that is located west of his home. He said that the odor has decreased and his dogs are no longer getting sick. However, Steve has been diagnosed with leukemia resulting from benzene poisoning. I suggested that he call the Stark County Health Dept. in regard to his medical condition. I called Greg New and informed him of Steve Clarks condition.

1238527368 2009-03-31 Production Wells No Violations I received a complaint call on 3-30-2009 from Steve Clark (330-701-7852) at 4000 Beechwood Ave., Alliance, OH, regarding a strong gas odor coming from the tank battery that is located west of his home. He said that gas keeps blowing from the separator and that he has been sick for 10 months and is getting checked for methane poisoning. I called Greg New who is taking care of this well for Cathy Lesher (C & T Energy) and discussed the complaint. On 3-31-2009, I inspected the tank battery. There was no gas leak observed at the separator. Gas was observed venting from the 2 dia vent pipe located on the oil stock tank. I called Greg New and requested that he place a charcoal filter on the vent pipe. I called Steve Clark and informed him of my inspection and recommendations.

1242505406 2009-05-15 Production Wells No Violations I received a complaint call at 0845 hrs from Steve Clark (330-701-7852) at 4000 Beechwood Ave., Alliance, OH, regarding the odor at the stock tank at the tank battery that is located west of his home. He said that the odor has increased and that he has a headache and his dogs are getting sick. I called Greg New and requested that he send the pumper out to check the separator and the charcoal filter. Greg called me back and said that the separator dump valve was stuck open and that the charcoal filter was plugged. The problem was resolved. I called Steve Clark at 1120 hrs and informed him that the problem was resolved. Steve was at the hospital getting checked for his health concerns.

The tank battery consists of two steel production tank, and a vertical separator. There is a domestic gas connection at the tank battery. The tank battery is shared with well #151-25540. The well is legibly identified. No compliance issues are noted at this time.
