0012091254 2014-07-21 Casing String Witnessed No Violations I was on location to witness the cementing of the 5.5/4.5 tapered casing string. A cement port collar was present at 4550 and cement was pumped through it using 2 3/8 tubing. 260sks of a lead cement of Unifill Light were pumped and followed with 100sks of 50/50 2% gel 10% salt tail cement. The cement was displaced with 17bbls of freshwater and the tubing was pulled from the well. The well will be bond-logged tomorrow to determine cement fill up. There are no violations at this time.
0261176074 2014-06-19 Surface Facility Construction No Violations I was on location to check the status of Pep Drilling rigging up on this location. A silt fence and retention pond has been installed on location to handle the large quantity of rainwater that has accumulated on the location. Trucks of rock have been continually brought onto location to stabilize the location. The rig is set in place but is still being retrofitted to drill the horizontal section of this well. The pits are planned to be dug today but may be delayed until tomorrow due to the water and mud. They expect to spud the well Saturday.
0289561688 2014-06-30 Drill / Deepen / Reopen No Violations I was on location to check the status of Pep Drilling this well. Currently the rig is drilling a 7 7/8 hole at 5000. They are currently building their curve for this well and will continue to slide and rotate drill until they reach their landing point.
0328808984 2014-07-29 Completion Testing No Violations I was on location to witness the hydraulic fracturing of this well. The first stage of the well began treatment when 800bbls into the frac there was an instant and unexpected surge of pressure and drop in treating rate. The treating rate dropped to 2 barrels per minute and the pressure spiked to 6400psi before the frac was shut down. The well was attempted to be surged to try to free any blockage of the frac ports. The well was pumped on and pressure released several times before the job was abandoned. There is an unknown fault with the toe frac port that is preventing any fluid from moving in the well. They are unable to drop another ball into the well to open another frac port because the ball needs 10bpm to seat. They are going to shut down for the day and attempt to bring in a coiled tubing unit to perforate the casing to allow the frac to continue.
0677378562 2014-07-10 Drill / Deepen / Reopen No Violations I was on location to check the status of Pep drilling. The rig is currently about to trip back into the hole at 6994 after replacing the drilling bit and down hole motor. They expect to TD the well this weekend and cement early Monday. There are no violations at this time.
0706790967 2014-07-30 Completion Testing No Violations I was on location to check the status of brining on a coiled tubing unit to perforate the well. The coiled tubing unit was awaiting permits from ODOT to be able to be transported from West Virginia. The plan is to run 2 tubing into the well through the 2.25 port in the packers for the well. The perforating gun and collars on the tubing are larger then the 2.25 diameter. The perforating gun will be able to fit through the ports in the well but the tubing will not be able to run through the port and only the first section of the casing will be perforated. The tubing units are expected on location tomorrow.
0710086878 2015-11-10 Preliminary Restoration No Violations I was on location to perform a preliminary restoration inspection of this well. The lease road is graveled and is stable. The tank battery is surrounded by a dike and properly identified. The tank battery contains four (4) 210bbl production tanks, vertical separator, chart box and all required locks and plugs. The well is an electrically driven pumpjack well with a downhole compressor system and all required gauges and locks. The location has been reclaimed, seeded and vegetation has been established. The wellhead, tank battery and roadway all conform to spacing requirements. At this time preliminary restoration has been passed.
0962560437 2014-07-14 Drill / Deepen / Reopen No Violations I was on location to check the drilling of this well. The Pep Drilling rig has been pulled off the well and is waiting to leave location for another well. The 4.5/5.5 tapered casing has been installed with 15 packers for staged frac job. A Chipco LLC rig will be moving on location to run tubing and cement the casing sometime next week. There are no violations at this time.

1387608203 2014-07-21 Casing String Witnessed No Violations On the date of inspection I accompanied ODNR Inspector Fred Romeo to witness the cementing of the production string. I was only able to witness part of the cementing job. Refer to Fred Romeos report for further details.
1402417102 2014-06-11 Surface Facility Construction No Violations I was on location to check the construction of this location. The lease road is built and graveled. A crew is currently installing an electrical pole at the road for the location. The pad has currently been graded and is currently only soil. Rock will be brought onto location and spread to stabilize the location. Silt fence and hay bails have been installed as erosion controls. There are no violations at this time.
1407351445 2014-08-06 Completion Testing No Violations I was on location to check the status of flowing this well back following the frac job. Currently the well is flowing back at 60bbls an hour and is not making any gas. A slight show of oil was present but not in large quantities. The flowback will coninue for several days before the well will be put into production. There are no violations at this time.
1407870823 2014-08-13 Completion Testing No Violations I was on location to obtain GPS coordinates for this well and to check the status of flow back for this well. The well is currently flowing back at 15-20 barrels per hour and has been steady. Completion personnel are not yet sure of how much longer flow back will continue. They are still selling any gas seen in the flow back through the existing sales line on location There are no violations at this time.
1801961046 2014-07-31 Completion Testing No Violations I was on location to investigate complaint #4759. The complainant stated that around the week of July 6 he noticed the ice cubes made with his well water were salty. This well is roughly 2500 from the residence. I checked the pits for the well and they have been closed and there are no signs of leaching. The cellar to the well is water tight as residual rain is sitting in the cellar and not dropping. The 11.75 and 8.865 cement jobs went without any issues and both had good cement returns to surface. The rig drilled the well with a super saturated brine and started drilling with it on 6/29. There were no losses in the well while drilling except when 175bbls were lost at 5185 in the shale right before the Clinton. The well is currently waiting to be hydraulic fractured. A coiled tubing unit is on location to perforate the toe of the well. There are no mechanical issues with the well and there are no violations at this time.
