The identification is located on the 100 barrel tank, the production records are current, and there are no violations present. |

The well was completed through the 4.5 inch diameter production casing with perforations from 1738 feet to 2019 feet in depth. The well was equipped with 7 inch diameter surface casing to 379 feet and 4.5 inch diameter production casing to 2076 feet. G-R Contracting was rigged up on location with their derrick service rig to complete plugging operations. Formation Cementing set up on location and pumped a bottom plug through the 1.5 inch diameter tubing from 2049 feet to 1649 with 30 sacks of class A cement with 1.18 yield. G-R tripped the 1.5 inch diameter tubing out of the hole. Following the plug Appalachian Well Surveys set up on location to bond log the 4.5 inch casing and shoot the casing off in order to retrieve the casing. Appalachian ran a bond log on the 4.5 inch diameter casing and found that the backside of the 4.5 inch diameter casing cement top was at 1090 feet. The bond log also recorded multiple cave ins on the back side of the 4.5 inch casing from 680 feet to 650 feet, 510 feet to 480 feet, and 430 feet to 410 feet. Appalachian shot the casing off at 405 feet in depth and tripped out of hole with the wireline. G-R pulled up 5 feet on the 4.5 inch casing to remove a 5 foot pup joint and to verify that the casing was free. |
