0160057681 | 2019-01-23 | Production Wells | No Violations | The purpose of my inspection was a follow up to an incident that happened on 1/21/2019 where a Enbridge pipeline had failed and caught fire causing damage to multiple resident dwellings and out buildings. There were multiple locations that were on fire, including several hay bales near the well head. On the day of my inspection I witnessed production equipment included a 100-barrel steel production fluid storage tanks, and vertical separator enclosed within an earthen dike. There was a domestic gas line attached to the flow line near the separator, and the gauge on the separator was leaking at the time of my inspection. The well was equipped with 4.5-inch diameter production casing, 1.5 inch diameter tubing with rods and National pumping unit, there appeared to be another domestic gas line off of the 4.5 inch diameter casing, the 1 inch diameter line was equipped with a tee, it appeared that one side of the tee had provided natural gas for a motor on the pumping unit, there was no longer a motor on the unit, the other side of the tee appeared to have plastic line attached that had been melted. There was a small leak on the 2-inch diameter check valve that was equipped on the tubing. At the time of my inspection all valves were shut. |

1548171641 | 2019-01-22 | Production Wells | No Violations | On site inspection as a follow up to a pipe line failure that occurred on 01/21/2019 when a 30 inch diameter gas line reportedly owned by a Midstream operator identified as Enbridge failed in close proximity to the well head and the escaping product caught fire. The resulting fire and radiant heat damaged or destroyed multiple structures in the affected area and the said well head along with the pumping unit showed signs of heat exposure from the event. The well was observed to be equipped with a 4.5 inch diameter casing head with 1.900 inch diameter production tubing inserted into the well head. The well is set up to be produced by the means of a pumping unit with a polish rod liner installed into the 1.900 production tubing. The internal seals on the polish rod liner appear to of been compromised by the fire, no drive motor or engine appeared to be present on the pumping unit. Both the casing and tubing well heads are equipped with 2 inch diameter production outlet ports. Installed in the 2 inch diameter outlet ports are 2 inch diameter flow lines that is equipped with both shut off and check valves. The shut off valves are closed and both check valve bodies were leaking around the upper cap area. Opposite of the commercial side of the 4.5 inch diameter well head is the service side that consists of a 2 inch diameter outlet reduced down to a 1 inch diameter sales line with a pressure reducing regulator present inline. The 1 inch diameter plastic flow lines attached to the service side have been burnt off, a 1 inch production valve is present on the service side and is in the closed position. The 4.5 inch diameter casing and surface casing well head section is not visible above ground level. On location in proximity to the well is the production facility that consists of a single 100 barrel steel production tank along with a mechanical separator. The well was observed to be equipped with 4.5 diameter production casing and 1.9 diameter production tubing. All production valves were closed. The well head leaks previously noted during on our last inspection on 01/22/2019 have subsided. The production facility was leaking around a pressure gauge on the mechanical separator during our last visit and that gauge has been replaced since our last visit. Inspector Armstrong and myself discussed with Mr. Smithingell the needed repairs to the well head due to heat damage from the fire during the pipeline incident. Mr. Smithingell has indicated that the pipe line company (Enbridge) will sit down with the landowner who owns the well and discuss the needed repairs and work with the landowner to address the damaged well head and equipment. They will contact me after the meeting with an update which is scheduled to take place in the next 7 days. |

The flowlines are joined at the well head into a single steel 2 diameter line that runs to a nearby production facility separator. Present on the separator is a pressure gauge showing 20 pounds pressure on the gauge. The domestic supply is present on the separator. There is a single 100 barrel steel tank present. Also on this day I was informed by Enbridge officials that a local well service company identified as Harmons Well Service is slated to move in on February 25 and make repairs to the well and replace any equipment damaged by the fire. |

The production equipment had been re-set and leveled as well as painted and identification was posted on the production fluid storage tank. It appeared that there was a domestic gas hookup from the separator utilizing a little joe regulator. The well was equipped with 4.5-inch diameter production casing, 1.5-inch diameter tubing with rods, a pumping unit with a natural gas motor. There was a new stuffing box, brass liner, and all new valves installed at the well head. The well head and pumping unit had also been re- painted and the pumping unit had been leveled with new timbers installed. |
