During my inspection the well was equipped with 4.5 inch diameter casing with a tubing head, 1.5 inch diameter tubing, rods, and a pumping unit. The production valves were in the open position. The production equipment consisted of one 100 barrel tank, one vertical separator, and one meter run with a 2 inch diameter line, .1875 inch orifice plate, and chart meter with a chart dated 7/31/17. Identification was posted at the wellhead and on the 100 barrel tank. |

The wellsite was located within an American Electric Power construction site that consisted of a gravel pad around the wellhead and a gravel road to the well. American Electric Power plans to set a transmission electric pole near the plugged well. The location of the plugged well did not have any erosion issues and there were not any remains or debris left behind from plugging operations. |

Formation Cementing set up on location and pumped a bottom plug through the 1.5 inch diameter tubing from 2170 feet to 1770 with 30 sacks of class A cement with 1.18 yield. G-R tripped the 1.5 inch diameter tubing out of the hole. Following the plug Appalachian Well Surveys set up on location to bond log the 4.5 inch casing and shoot the casing off in order to retrieve the casing. Appalachian ran a bond log on the 4.5 inch diameter casing and found that the 6.25 inch diameter hole had caved in from 950 feet to 1050 feet in depth creating a 100 percent bond on the 4.5 inch diameter casing. Appalachian shot the casing off at 900 feet in depth and tripped out of hole. G-R attempted to pull up on the 4.5 inch diameter casing but could not do so. Appalachian and G-R reviewed the Bond log again and decided on shooting the casing at 610 feet. Appalachian tripped back in hole with the casing cutter and successfully shot the 4.5 inch diameter casing at 610 feet in depth. G-R tripped one 32 foot joint of casing from the well. |

During my inspection G-R Contracting was rigged up on location with their derrick service rig. I spoke with Jordan Jones with G-R Contracting and he stated that the well shall be plugged due to an American Electric Power electric transmission line being placed directly over the well. G-R Contracting tripped the tubing out of the hole and ran back in the well open ended preparing to plug. No further action will be taken until a plugging permit is received from ODNRs permitting section. |
