Also, a Quantab test was used to test the brine water. The high range Quantab went off scale showing that the water was greater than 6000 parts per million chloride. At 10:02 am we telephoned the company to inform them and the first company man was on location within ten minutes to shut the well down. We met with Scott Sterling on location and another man named Jim. They placed buckets by the well head to catch the leaking brine. They informed us that the well would remain shut down until the tubing could be pulled and repaired. |

At 10:02 am we telephoned the company to inform them and the first company man was on location within ten minutes to shut the well down. We met with Scott Sterling on location and another man named Jim. They placed buckets by the well head to catch the leaking brine. They informed us that the well would remain shut down until the tubing could be pulled and repaired. |

Jeff Watson, Dave Hackenberry, and Larry Marks were all on location. The tank battery dike exceeds that of what was recommended by the Division. The recommended size was 125 feet by 27 feet by 2.5 feet deep. The finished dike size ended up being approximately 140 feet long, 30 feet wide, and about 3.5 feet deep. There were four inches of sand placed below and above a 30 mil one piece liner. An additional liner was placed on this top layer of sand and another four inches of sand and screenings were placed upon this second liner. Four new 300 barrel tanks built by Roads Tanks are being placed inside the dike with new scaffolding. A-51 tar was painted on the bottom of the tanks. SUR-11 plastic piping will be fused together for the line leading to the tanks from the unloading pad. Many photographs were taken of the construction process. Also, samples of the two liners, sand, and screenings were taken. The injection well was also tested. There was 310 psi on the annulus and 840 psi on the injection. The well was running at the time of the testing. |

An inspection revealed a 20.9 section of tubing broke off (not at collar) from remainder of string and was observed on the ground. A 7 7.5 section was cut off and now only 2 10.5 section of tubing extends above wellhead. A gauge placed on the injection side of well showed 20 psi and 10 psi on the annulus. A review of pressure chart at the well location, showed an abrupt decline of both injection and annulus pressures at time of incident. The injection pressure dropped off from 1000 psi and annulus psi of just under 600. Maximum injection pressure was never exceeded. Crew and equipment at location and have begun cleanup. The immediate area surrounding this well has been excavated and placed in a pit, which contained a liner. The impact extended from the well location to the East approximately 200 (length) into a low lying field with pockets of standing water near Priest Run. Note: Rainfall events this week have saturated the ground. A dam was built, in the field, to prevent entry into stream from a ditch line. A water truck removed fluid from an area with pooled water away from this stream approx >50 containing Cl in standing water. Quantity released is unknown at this time. No standing crude oil was observed. A truck load of lime in bulk arrived at location and began applying to area in field at 11:30 am. After application of lime, straw mulch, organic material, seed ( ie cereal rye) and straw mulch will be applied to this area. Mr. Priest agreed to perform additional bioremediation in the Spring, if impacted area does not respond to initial treatment. Note: Attempts also being made to schedule service rig to perform repair. |

The company agreed to remove pooled fluid and opted for bioremediation of impacted areas with the application of lime, horse manure, seed. mulch and tillage in drier weather. Although not required for this facility, the construction of an earthen dike around the well was recommended to prevent future offsite impact. |
