0330426004 2016-06-20 Production Wells No Violations On the day of my inspection I found that the disturbed area around the wellhead was growing with thick vegetation. The leaks at the control box and change over nipple had been repaired. There was no oil on the ground around the wellhead. The permit number was posted on the control box. The dike had been cleared of all contaminated soil and filled in with new gravel. This violation is resolved.

1454341660 2016-02-01 Production Wells Violations Noted On the day of my inspection I found that the wellhead was equipped with a Concoil lift system. The tank battery consisted of two 100 barrel tanks, one separator and an earthen dike. Identification was posted. The tank battery is shared with permit 6548. I found a 30 x 4 swath of crude oil that had been sprayed from a valve on the Concoil into the brush an onto the ground along the lease road. I called Mark Fluharty who claimed that new pumper caused the mess. He said that he would be out to remove the contamination within two or three days, and he would notify me when it was cleaned up.
1455031416 2016-02-09 Production Wells No Violations On the day of my inspection I found that the affected soil and vegetation had been excavated and removed from the site. This violation is resolved.
1456169033 2016-02-22 Production Wells Violations Noted This inspection was conducted in response to a complaint of alleged pollution and contamination. I was joined by SE Regional Supervisor Brent Bear and NE Regional Emergency Operations Manager Terry Whitehair. On the day of our inspection we found that the well was being produced with a casing plunger system. There was no identification posted at the wellhead. The well was leaking oil from the change-over nipple below the ball valve. There was also oil on the ground from a leak where 3/8 tubing connected to the control panel. There was a recent oil spill at this well and the complainant claimed that the contaminated soil and vegetation had been buried on site. I spot checked the freshly graded soil in five locations sound the impacted area and found one spot where there was a minimal amount of oil soaked leaves and roots buried 6 to 8 below grade. The tank battery consisted of two 100 barrel steel tanks and one horizontal separator. An earthen containment dike was in place around the production equipment. The tank battery is shared with permit 6548. Identification was posted at the tanks. There was an area of oil/brine impacted soil mixed with sawdust under the load valves on one of the 100 barrel tanks. There was also a pile of oily sludge lying on the outside of the dike wall. The issues needing addressed at this well to bring it into compliance include: posting identification at the wellhead, correcting the leaks at the wellhead, removing and properly disposing of the impacted soil inside of the dike, removing and disposing of the oily sludge pile outside the dike, bio-remediate the impacted soils at the wellhead and establish vegetative cover.
1459189032 2016-03-24 Production Wells No Violations I was contacted by Mark Fluharty, pumper for Northwood, to ask if I could meet him at the location in order to inspect the work he has done to bring the well into compliance. On the day of my inspection I found that the oily soil and sawdust had been removed from inside the dike. He had excavated down to the clay layer. I told him that he could fill in with new gravel. The oily sludge pile had also been removed from the dike wall.
