During my inspection the well was equipped with a plunger lift system with production valves in the open position. The production casing had a gauge present reading 300 pounds per square inch and the production tubing had a gauge present reading 182 pounds per square inch. The production equipment consisted of fourteen 400 barrel tanks, 8 gas production units, 4 compressors, and 2 combustors. The natural gas pipeline tie in was located on the south side of the pad. The identification was located on the well and the 400 barrel tank. |

Upon inspection on this day I met with Anteros HSE representative Chris Warnock and Myers Well Service Regional Safety officer John Moore who indicated the employee of Myers Well Service was injured when a 3 line used in the production fluid transfer process was attempted to be disconnected from the water truck while under pressure causing the line to release from its connection and struck the employee in the face causing lacerations and fractures to the face. The line was free from fluid and no well bore related product was released. The company(Antero) has implemented a safety stand down until the incident can be investigated and corrections can be made to further avoid any future incidents. Addressing the well location a total of 8 wells were found on site and in production with a total of 14 production tanks present with a total capacity of 400 barrels each. All tanks were found inside of a poly lined metal construction containment dike area. Also on site there is a total of 8 gas processing units one supporting each existing well on the location. With proper well identification posted at the well head. Myers Well Service Ohio UIC # 25-427 water trucks were on location along with Ohio Oil Gathering hauling production fluid from the site. |
