0138831709 2016-06-09 Final Restoration No Violations On the day of my inspection I found that the well site, access road and tank battery site had been graded out and seeded. The access road and well site were growing with well established grass. The tank battery site was still mostly bare. I found no oilfield materials or debris left behind. I called and left a message with Dean Limbacher with NGO to re-seed the area where the tank battery was located.
0142309111 2015-09-21 Plug / Plug Back No Violations On the day of my inspection I found that RB Drilling was moving in and rigging up to plug the well. I found crude oil inside of the dike. I called Dean Limbacher of NGO and told him that they will need to remove the contaminated soil from the tank battery before they reclaim the site.
0198453147 2015-09-23 Plug / Plug Back No Violations On the day of my inspection I found that RB Drilling had tubing ran to 4519. Cemenco pumped 35 barrels of fresh water ahead to establish lift pressure prior to pumping the plug. They pumped a 30 sack cement plug through the Clinton sand. RB Drilling pulled the tubing from the well. They could not get a telegraph when they tried to rip the 4.5 casing. They will have the casing shot off tomorrow. I also found that NGO had an excavation crew on location to remove the contaminated soil from inside the dike.
0494913292 2016-07-14 Final Restoration No Violations On the day of my inspection I found that new top soil had been brought in and spread over the old tank battery site. The area had been seeded and covered in straw. The grass was starting to come up, but the coverage was minimal.
0731691388 2015-09-28 Plug / Plug Back No Violations On the day of my inspection RB Drilling had tubing ran to 2090. Cemenco established circulation and pumped a 150 rip plug. RB Drilling pulled tubing up to 1259 and Cemenco pumped a 200 plug at the bottom of the surface casing. RB Drilling pulled the tubing up to 200 and Cemenco pumped until cement was circulated to surface.
1443017161 2015-09-22 Plug / Plug Back No Violations On the day of my inspection RB Drilling pulled the tubing from the well and ran back in to set a Clinton plug. I met with the service rig operator Derrick Buxton to discuss the proposed depths of the cement plugs and where to start ripping the 4.5 casing. I also found that the standing water and oil had been vacuumed from inside the dike and the soil had been covered in Bio-sorb.
1443107833 2015-09-24 Plug / Plug Back No Violations On the day of my inspection Appalachian Well Surveys shot the 4.5 casing at 3200. RB Drilling tried to pull and the casing would not move. Appalachian shot it again at 2900 and the casing still would not pull. Appalachian ran a bond log and found that the hole was caved in around the casing from 2270 to 2278 in the Ohio shale. RB Drilling ran back in with tubing to 3180 in order to squeeze a cement plug in the top of the Big Lime.
1443536400 2015-09-29 Plug / Plug Back No Violations On the day of my inspection NGO cut the 8.625 casing off 48 below grade and capped it with a steel plate that was inscribed with the permit number. I also found that they had closed the pit, removed the production equipment and graded the tank battery site.
1480604878 2016-12-01 Final Restoration No Violations On the day of my inspection I found that the tank battery site was completely covered in short grass. There were no bare spots and no erosion. The well site and lease road were found to be restored during my inspection on 6/9/16. This site passes final restoration.
2065756964 2015-09-25 Plug / Plug Back No Violations On the day of my inspection RB Drilling had tubing ran to 3180. Cemenco established circulation and pumped 86 sacks of class A cement. RB Drilling pulled the tubing up to 2032 and Cemenco reverse circulated 12 barrels to clear the tubing. Cemenco then pumped through the tubing to squeeze cement into the shots at 3200 and 2900. They pumped until the pressure increased to 800 psi, shut it in for 5 minutes and released. RB Drilling pulled the tubing and went in to rip the 4.5 casing above the tight spot at 2270. They got the casing ripped off at 2112, pulled it from the well and ran tubing to 2090 to set a rip plug.
