1045914033 2017-05-16 UrbanProduction Wells No Violations I inspected this well location as a follow-up to Chiefs Order # 2016-171. At the time of this inspection, I observed the following: a wellhead with plunger-lift system and domestic metering connection at the 4.5-inch casing. The connections and fittings at the wellhead were in good condition and well maintained. The battery was comprised of one 25-barrel steel, stock tank and horizontal separator. The well was in production and the charter recorder was active and in-use. There was no identifying signage at any place on the well system. I did not observe any signs of impact to the surrounding areas. The short access road from Call Road was mostly grass-covered but in good condition. Per my discussion with Mr. Svetlak on 05/16/17, the well was in active production at the time of this inspection. There has not been an annual production report filed since 2003.
