1602323158 | 2016-02-22 | Production Wells | Violations Noted | This inspection was conducted in response to a complaint of alleged pollution and contamination. I was joined by SE Regional Supervisor Brent Bear and NE Regional Emergency Operations Manager Terry Whitehair. On the day of our inspection we found that the well was being produced with a natural gas powered pumping unit. The stuffing box was leaking. The wellhead was covered in crude oil and there was an area of crude oil contamination in a 5 foot diameter around the wellhead. The lease road up to the well was washing out. The tank battery consisted of one 100 barrel steel tank and one 30 barrel poly tank. An earthen containment dike was in place around the tanks. Identification was posted at the tank battery. The soil under the load valve on the 100 barrel tank was damp and stained from fluid being released through a bleeder valve when the tank is drained. There was a horizontal separator uphill, about halfway between the well and the tanks. We also found a 50 barrel steel tank outside of the dike that was not plumbed to the well or tank battery. The tank contained drip gas condensate. The issues needing addressed at this well to bring it into compliance include: draining and removing the 50 barrel condensate tank, removing and properly disposing of the impacted soil inside of the dike, removing and disposing of the impacted soil around the wellhead, addressing the erosion along the lease road. |

1979405278 | 2016-03-17 | Production Wells | No Violations | I received a phone call from Mark Fluharty, pumper for Northwood, to inform me that he was working on the clean up at this well. He asked if I could meet him out there to provide some guidance on what was specifically required. When I arrived on location I found that the wellhead and related components had been cleaned of all crude oil residue. The oil that was on the ground surrounding the wellhead had also been removed. Mark had dug down approximately 12 to 15 around the wellhead. There was some residual staining in the soil and gravel, but there was no standing oil or fluid. The area surrounding the wellhead was also stained. I had Mark dig down approximately 3 and found a layer of clay. I told him to remove the top layer of stained soil and gravel until he got down to the clay layer. Then he must treat the area with lime and peat/fertilizer before adding any fresh gravel. I also found that he had cleaned the engine and removed most of the oil soaked gravel that was under the engine. I told him to get as much out as possible and treat the area with lime and peat. Once he replaces the gravel I recommended that he keep clean absorbent pads under the engine as a preventative measure against future leaks. I instructed Mark to remove the soil in the dike that had been contaminated with brine and fill the area in with fresh gravel. I also suggested that he attach a small hose to the bleeder valve and run the water into a bucket when he is having the brine hauled. |
