1350305554 2012-10-15 Production Wells Violations Noted Inspection of the well head found a older producing well set up to be produced by the means of a pumping unit with a single 100 barrel tank set approximately 30 from the well head with obvious signs of surface contamination from crude oil below the tank. Several violations present that need addressed are as follows. The 100 barrel production tank set back 50 from the well, dike needs built around storage tank, report production to the division, identification needs posted, and production fluid contamination on the ground below the 100 barrel tank needs cleaned up.
1356543825 2012-12-26 Production Wells No Violations Compliance agreement follow up inspection found that the well is still idle and the 100 barrel tank still leaking with no sign of recent activity around the well head or tank battery. Notification was made to the company main office that further action will be taken if the terms of the compliance agreement is not met.

Upon inspection of the well head it was observed that the well has no production casing flow lines present and the current condition of the pumping unit makes it incapable of commercial production. The well tender for the company has been advised of the unresolved violation and idle well.

1477682640 2016-10-28 Production Wells No Violations Inspection follow up to Chiefs Order # 2016-346 and Compliance Notice # 1350305554. On this day it was observed that the well was equipped with 4.5 production casing - 2 production tubing and pumping rods with no flowline attached to the casing and no commercial gas sales meter in the area associated to the said well. There is no domestic hook up present at the well head and the landowner stated she is not using the well for domestic purposes due to the wells inability to supply gas in proper quantities. The 2 tubing is hooked up and the well is set up to be produced by the means of a pumping unit, there are no drive belts on the pumping unit and the electric service meter has been removed. The production tank is located at a improper setback distance from the well head as addressed in previous reports with no attempt being made by the producer to correct the set back issue. Dense vegetation is present around the well and production tank. During the last 2 weeks a Mike Kendall has contacted me representing B&N Coal and inquired about transferring the well in which I referred him to the Divisions Legal staff, Mr. Kendall also indicated they would plug the well upon the receipt of the plugging permit that has been applied for. On this day I spoke with a Tyson Brock with B&N Coal who indicated they would not be plugging the well and were appealing the order. Photographs were taken of the well and production tank on this day.
1479298541 2016-11-16 Production Wells No Violations Order # 2016-346 follow up inspection accompanied by Division Supervisor Brent Bear and Ohio Attorney General Scott Myers. Upon inspection the following diameter casing strings were observed as present at the well head 11-9.625 6.625 4.5 along with a 2 tubing string which is equipped with a pumping tee and pumping rods. There is a 2 sales line feeding into a 100 barrel production tank set at a measured distance of 184 from the well head. The well is equipped to be produced by the means of a pumping unit and the electrical service to the well has had the meter removed. No gas sales line is attached to the well. The landowner identified as Bud Weddle was on location and indicated the well has not provided him with domestic gas for 2 years.
1479608480 2017-03-29 Plug / Plug Back No Violations Inspection follow up to Chiefs Order # 2016-346 and Compliance Notice # 1350305554. On this day I was on site along with ODNR Inspector Brian Sefcik and witnessed Harmons well service rig on the hole pulling the 2diameter production tubing. The rig recovered 2,058 of 2 diameter 10 round tubing. Appalachian Wireline Service ran a Casing collar locator/ Gamma Ray/ Variable density log with the log showing a total measured borehole depth of 2,027 and perforations scattered from 1,966 to 2,016 and perforations between 1,692 & 1,700. The top of cement behind the 4.5 was marked at 1,465 and the bottom of the 6.625 was marked at 1,030. There is also 2 additional casing strings present at surface that measures 8.626 and 10.625 in diameter. After logging the well the rig began running the 2 tubing back in the hole to bottom so a 500 bottom plug may be set the following day to cover all perforations.
1489418067 2017-03-13 Administrative Inspection No Violations Administrative follow up Inspection to Chiefs Order # 2016-346 and Compliance Notice # 1350305554.. On this day Harmons Well Service notified me that they would be moving in the week of 03/20/2017 to begin plugging operations on the said well.
1490884469 2017-03-30 Plug / Plug Back No Violations On site inspection to witness the well plugging process with Harmons Well Service rig on the hole and Formation Cementing service. The well is equipped 4.5 diameter production casing that was logged by Appalachian Wireline on 03/29/2017, the log showed perforations in the Gordon Sand from 2,016 to 1,966 and perforations in the Berea Sand from 1,692 to 1,698. Harmons Service rig has installed 2 diameter 10 round thread tubing to a measure depth of 2,044 and Formation set a 500 8.121 barrel bottom plug consisting of 38.6 sacks of class A cement, the plug was displaced to 1,544 with 4.59 barrels of fresh water, 7.5 barrels of 6% fresh water gel was ran ahead of the bottom plug. The 2 tubing was pulled and the bottom plug tagged at 1,467 with the casing ripper before ripping the 4.5 at 1,056and setting a 10.5 barrel 300 plug consisting of 50 sacks of class A 1% gel 1% calcium cement displaced to 756 with 12 barrels of fresh water and the well circulating.
1490968059 2017-03-31 Plug / Plug Back No Violations On site inspection to witness the final upper section plugging process with Harmons Well Service rig on the hole and Formation Cementing. On the previous day a 300 plug was set through the 4.5 after the casing was ripped at 1,062. We ran in this morning with the casing ripper and the cement top of the 300 plug was tagged down hole at 1,070. After re evaluating the gamma ray log the well appeared to have a shot hole from 1,062 to 1,110. The rig then ran back in the hole with 2 diameter tubing to a depth of 1,062 and another 10.5 barrel plug was set consisting of 50 sacks of class A cement, with lift 250 psi lift pressure established and maintained during the pumping of the job and the well bore circulating. The 2 was pulled and the 6.625 casing ripped at 340 and pulled out of the hole. The rig then ran the 2 diameter tubing back in to 300 and Formation pumped a 22.4 barrel plug consisting of 100 sacks of class A cement with the well circulating fluid but no cement. At that point a second cement bulk truck was used and an additional 10 barrel 45 sack plug was pumped with cement to surface. Cement also circulated up between the 8.625 and 10.625. The well was originally drilled by cable tool and at some point deepened by a rotary drilling machine.
1491041869 2017-04-01 Administrative Inspection No Violations Administrative Inspection follow up to Chiefs Order # 2016-346 and Compliance Notice # 1350305554 to document the well plugging process was complete on 03-31-2017. All previous violations are considered resolved and the plugging report submitted with all plugging records.

1797139626 2017-03-30 Plug / Plug Back No Violations I was on location with Inspector Bill Stolarik for a training opportunity on plugging a well. Harmon Well Services was rigged up on the wellhead and Formation cementing was on site to cement the well. This well is equipped with a 4 1/2 inch diameter wellhead. Appalachian Well Services ran a variable density log on the well to find the perforations for this well. Perforations were found from 2016-1966 feet and 1968-1962 feet. Harmon well services ran 2 inch diameter ten round tubing into the hole to 2044 feet. Formation then set a 500 foot class A plug from 2044-1544 feet covering the Gordon Sand and Berea Sand perforations.
