0248043718 | 2014-07-01 | Production Wells | No Violations | Complaint # 4650. My inspection found there is no evidence the company has even attempted to comply with the compliance notice issued on 4-1-2014 and is due on 9-1-2014. I contacted Kirk Sherrill and left a message for him to contact me and update the status of plugging this well. |
0281121530 | 2014-04-01 | Production Wells | Violations Noted | I checked this well in response to a public complaint# 4650 from Margery Brown [ landowner ] claiming the well is idle and should be removed . My inspection found there is one 210 barrel steel tank and one separator at the tank battery location . The area inside the earthen impoundment wall around the tank and the separator is overgrown with small trees and brush . There is not a identification sign posted for this well at the tank battery or the well locations . Further inspection of the well head I found 4.5 casing with a swage to 2 with a valve which is closed . The well is not connected to the flow line . The RBDMS records showed the last reported production for this well was in 1999 and was only 6 million cubic feet of gas . This well is idle . REQUIRED REMEDIAL ACTION : PLUG WELL . I contacted the company and I spoke with Kirk Sherrill and told him of the compliance violations and that the well has to be plugged . He did not have a date for when they would plug the well . |

0754956017 | 2017-09-18 | Final Restoration | No Violations | My inspection of the Brown # 1 for final restoration revealed that the two (2) inch riser pipe had not been removed, the tank pad was still void of vegetation. I spoke with Matt Kleese, who said, that he will not work on a live line to remove the riser and does not know the lines owner or if there is a gate valve to isolate the riser and the material on the tank pad was placed there after the tank battery removal. He said, that he had that discussion with DOGRM personnel previously and wont proceed any further. The R & M Brown # 1 was not compliant with the Final Restoration rules. |

1284316319 | 2017-05-17 | Plug / Plug Back | No Violations | I was on site to witness the service rig go into the production casing with a drill bit on the tubing to attempt to drill through an obstruction in the well. They hit a obstruction at 2780 feet. They dry drilled on the obstruction for several minutes when they were able to drill through the obstruction. They run to bottom with the tubing. I left the site when they began pulling the tubing from the well. |
1423084298 | 2014-08-20 | Production Wells | No Violations | In response to a phone call from Margery Brown to Regional Supervisor Tom Hill, Inspectors Jerry Kohl, Josh Schiering and I met with Mrs. Brown regarding follow-up to Complaint #4650. Mrs. Brown voiced several concerns. She questioned why the idle well on her property was not yet plugged. We explained that there was an existing Notice of Violation (NOV) for the well requiring it to be plugged with a Required Action Date of 09/01/2014. We explained that if no action has taken place by that date, we would proceed with further legal processes following state law. Another concern was that she believed that unknown people were conducting oilfield activities on her property during the night, such as drilling new wells. She had heard noises and seen lights and people in or near the wooded area east of her home. We explained that no permitted drilling activities were taking place in Ashtabula County at the time of our meeting. We offered to do a visual survey of potential access points to her property to see if there were any fresh traffic patterns from the main roads onto her property. We also advised that if there were non-oilfield activities taking place on her property without her permission, she would need to contact a local law enforcement agency. Mrs. Brown also suggested that someone was taking gas from the idle well on her property. We told her we would re--south along the west side of her property. We also looked along Footville-Richmond Road, which is oriented east west along the south end of her property. There we |

1593562453 | 2015-05-01 | Production Wells | No Violations | Follow-up, Complaint# 4650, Marjorie Brown. I checked this well in response to a phone call from Marjorie Brown, she claimed some one had changed the chart on a gas meter by the tank battery area. My inspection found a chart had been changed on a meter run near entrance to the tank battery. The meter run is owned by Phil Dietrich. Phil has no connection to the well on Marjories property. The well showed no activity to plug the well. |
1710893990 | 2017-06-09 | Final Restoration | No Violations | I arrived on site to find the separator, tank ladder, heater tube stack were still at the tank battery area. The tank battery had not been final graded. There were three two inch pipes coming from below the surface where the separator had been located. I called Jay with J-Well, he told me they will complete the final restoration at the first of the next week. |

2069122144 | 2017-08-28 | Final Restoration | No Violations | My inspection found that the Brown # 1 plugged and abandoned well was not in compliance with the final restoration rules. Per Tom Hill the flow line was previously removed. I found a meter box, two (2) inch pipe riser north of the old tank pad and one (1) inch orange plastic piping partially buried in the access road. The tank pad was void of vegetation. The Brown # 1 was not approved for final restoration. |
