1131803821 2019-04-16 Production Wells No Violations On the day of my inspection I was accompanied to the well site by SE Regional Supervisor Brent Bear. We found that the well was equipped with production casing on a casing hanger. The production casing and surface casing were both open to the atmosphere. There was a down tree laying across the top of the production casing. There were no flow lines connected at the wellhead. We found no signs of fluid leaks from the well, and we did not detect the odor of leaking natural gas. There was no conspicuous access to the wellhead and there was no production equipment associated with the well. There was no well identification posted at the wellhead. The only production reports ever reported for this well are from 2008 to 2017, except for 2011, 2012 and 2015, which were omitted. All the reported years show zero oil or gas production. On 4/20/18 I met with the landowner Mary Mason, who said that she has owned the property for 16 years and its been in her family since the 50s. She said that she cannot remember anyone ever producing it or doing anything with it. She did say that she remembered the tanks being removed in 1987. I took updated photographs of the wellhead.

1511896022 2017-11-27 Production Wells No Violations The purpose of my inspection is due to a Follow--199. On the day of my inspection I assisted Chris Grimm on the inspection. The well was not found per the coordinates that are provided on the RBDMS. There were pieces of flowline lying all over the area.
1511964361 2017-11-27 Field Inspected, Well Not Found No Violations On the day of my inspection inspector Pat Shreve and myself searched the area for the well based on the GPS coordinates stored in our database. The well location was plotting in an open pasture area between the township road and forest. We found no signs of a wellhead or well casing. There were a few loose segments of rusted, 1.5 inch diameter steel line scattered around the general area. RAM Oil had another well in the area, permit 3633, that has been abandoned, capped and buried without being plugged. I will contact our surveyor Tom Smith and return to confirm the presence and location of the well.
1524251610 2018-04-20 Production Wells No Violations On the day of my inspection I was joined by ODNR surveyor Tom Smith to located the well. We met with the landowner Mary Mason, who said that she has owned the property for 16 years and its been in her family since the 50s. She said that there was a well in the forest, but she cannot remember anyone ever producing it or doing anything with it. She did say that she remembered the tanks being removed in 1987. Tom and I found the well in the forest, below the open pasture area where the well coordinates were plotting. The surface casing was open to the atmosphere and there was production casing, on a casing hanger that was also open to the atmosphere. We did not detect the odor of leaking gas and I found no signs of oil on the ground around the wellhead. I dropped a rock in the casing to estimate the fluid level, and found it to be at approximately 500 feet. I gave Mary the well permit number and advised her to research the lease chain on the property. RAM Oil has submitted annual production reports for 2008, 09, 10, 14 and 2016, but they show zero (0) barrels of oil and zero (0) cubic feet of gas for every year reported. I took updated GPS coordinates for the well and Tom flagged it with pink tape.
