0037187255 2015-10-30 Solution Mining Project No Violations I was on location to observe the plugging of the City of Akron # 60 ( SMP # 3 ). Universal Well Services ( UWS ) was the cementing contractoe and Key Energy Services ( KES ) was the plugging contractor. UWS established circulation and mixed a cement slurry consisting of 231 sacks of Class A cement through the 2.875 tubing. The cement slurry was displaced from 2430 to 1830. The cement slurry weight was verified by scale and weighed 16.0 ppg. No violations were identified.

1292916696 2015-11-17 Plug / Plug Back No Violations I was on location to witness the topping off of the 8.625 casing after falling back downhole approximately 30. Orville Ready Mix used an less than one (1) cubic yard of nine (9) sack grout to bring the cement top up to four (4) feet above grade in the monument ( above ground ) section of casing. R G Smith contractors were to install the metal plate with the required information. No violations were identified.
1446488936 2015-10-30 Solution Mining Project No Violations I was on location to witness the pressure test of the 9.625 casing and the bridge plug set at 2430 GL. Universal Well Services ( UWS ) was the pumping contractor. A review of the Cement Bond log indicated sufficent cement bond to the formation to proceed without remedial action. After connecting to the 2.875 tubing UWS pumped approximately 10 barrels of fresh water to load the tubing and establish circulation. The annular Blowout Preventor was closed on the 2.875 tubing effecting a closed system to test the 9.625 casing. The test required a minimum pressure of 500 psi. with an allowance of Five ( 5 ) per cent pressure leak-off. UWS pressured up to 520 psi @ 1226 hrs. and allowed the pressure to stabilize. Subsequent pressure reading were as follows: 509 psi @ 1229 hrs., 505 psi @ 1231 hrs., 502 psi @ 1236 hrs. and 499 psi @ 1241 hrs. Total pressure decline was 21 psi, which equals a 4% pressure loss. The test for mechanical integrity of the 9.625 casing passed. Cargill Inc, was allowed to proceed with the plugging of the solution mining well. No violations were identified.
