At the time of this inspection, I observed the following: The wellhead plunger lift and associated tank battery located on a wooded parcel along Mayfield Rd. The load lines were plugged and secure. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery, and found no signs of impact or degradation attributable to the production system. The egress and ingress road was grassy but passable.

At the time of this inspection I observed the following: a wellhead with plunger lift system and associated tank battery in separate board-on-board enclosures which were locked and secure. The valves and connections at both the well and battery were secure and free from signs of leakage. I noted the following maintenance issue: some portions of the existing 2-inch flow-line fitting displayed significant corrosion. Process safety equipment was present. ID signage was present and accurate. The areas surrounding the well and battery were comprised of a wooded area on the east flanks of the Chagrin River, which is located 1/2 mile west. I walked the surrounding areas and found no signs of impacts or degradation. Access to the well and battery was afforded by shared-use, asphalt-paved driveway which was in good condition. I notified Ohio Valley Energy Systems Corp, about the need for maintenance at the wellhead.

1403111250 2014-06-18 UrbanProduction Wells No Violations I inspected this producing unit as part of regular site check and inspection activity. The well was completed on 10/09/2007. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger lift system. The wellhead is located in a closed and locked board-on-board enclosure. The 8.625-inch casing is valved to surface but is not gauged. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located near Mayfield Road and adjacent to the access road to the well, which is located 200 feet north. The battery is comprised of 1 steel 100-barrel production tank, 1 50-barrel poly brine tank, and a horizontal separator. The battery is located in a separate board-on-board enclosure which is gated and locked. The load-outs are bull-plugged and locked. There is process safety equipment in use (carbon filter, low-pressure relief valve, and spark arrestor). The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are noted to be in poor condition and need replaced. The wellhead and battery are located on a narrow benched areas with a significant topographic drop to the Chagrin River, located 1/2 mile west. I did not observe any signs of gully formation or washouts. Access to the unit is afforded by a dedicated gravel-paved access road accessed directly from Mayfield Road. The access road is currently in good condition and free of washout or erosional deficits. Beyond the area cleared for the wellhead and tank battery, the areas surrounding the unit are heavily wooded with one single-family residential home bordering to the south, and no other residential strucutres within eyesight. In contacted Ted Hill with Ohio Valley regarding the ID placard who indicated it would be addressed within the next 2 weeks.

At the time of this inspection, fittings and connections at the wellhead plunger-lift and battery were secure and free from leakage or problems. The individual enclosures were locked and secure. Production reports were current. Proper bonding and insurance was in place. The surrounding areas were covered in several inches of fresh snow cover.
