0396355729 | 2014-11-14 | Storage Well | No Violations | This is the yearly inspection for this well. The well is in working order at this time. The identification for the well is located at the wellhead. There are no violations at this time. |
0451846818 | 2013-10-04 | Storage Well | No Violations | This well is a storage well for natural gas. It has 3 casing in it. There are no leaks around the well. The identification is located at the well. No violations at this time. |
0850506343 | 2016-12-07 | Plug / Plug Back | No Violations | I was on location today to witness the plugging of this well. Today Appalachian Well Services came out to the well to run a bond log on the well before they shot the 5.5 inch casing off. They tried to shoot the casing off at 1900 feet but when the rig crew hooked onto the pipe it would not budge. Once Formation Cementing arrived on site we hooked them up and tried to circulate the casing free. The fluid never did return to surface after 70 barrels of pumping on it. While we were trying to work the casing free the top joint of 5.5 inch casing popped out of the threads. They tried to get a new 5.5 inch sub but they could not get screwed back in the old threads. The operations had to stop for the day so they could get a different tool for tomorrow to hook onto the pipe. |

1167739650 | 2017-04-19 | Final Restoration | No Violations | On my inspection today, I am accompanied by inspector Smith to conduct a final restoration on well site. Final restoration meets 1509.72b requirements. |
1288866713 | 2015-06-18 | Work Over Reconditioning | No Violations | I stopped by this well to check on the job status. Kenny Wright has his service rig on location doing a work over on the 3 casing. Baker Tools lost a cutter down the well and Kenny is working on fishing it back out. |
1435683479 | 2016-04-05 | Storage Well | No Violations | I was on location to do a field inspection on this storage well. The well is not currently hooked up. The well is being used to monitor the field. |
1439820806 | 2015-08-17 | Storage Well | No Violations | At the time of my inspection wellhead is shut in and disconnected from the meter run. There is a pipe trailer and 110 BBL tank that have been moved in on location. I have been informed that NGO Transmissions is debating on plugging or servicing this storage well. |
1480960259 | 2016-12-05 | Plug / Plug Back | No Violations | I was on location today to witness the plugging operations of this well. Kenny Wright has moved his service rig in on the well to plug it. The bottom plug will be pumped through the 2 inch tubing with 50 sacks of Class A cement from Formation Cementing. After the bottom plug was set the rig crew pulled the tubing up out of the cement then Formation Cementing hooked up their kelly hose to hold pressure on the cement for 30 minutes. The rig crew then worked on pulling the tubing out of the well. |
1481307738 | 2016-12-09 | Plug / Plug Back | No Violations | I was back on location today to witness the plugging of this well. Today Formation cementing was on location to pump a Big Lime plug from 1920 feet to 1520 feet. Once the plug was pumped the rig crew pulled the tubing out of the well so Appalachian could shoot the 5.5 inch casing off at the Berea. They were unable to get the pipe free at the Berea so the tubing was ran back in the well and another plug was pumped through the tubing at 937 feet to 637 feet. The tubing was then pulled back out of the well so Appalachian could run there tools back in the well and shoot the 5.5 inch diameter casing at 300 feet. Once again the pipe would not come free. I had Appalachian go in and shoot the 5.5 inch diameter casing at 200 feet. We were able to establish circulation at this point so we hooked Formation Cementing back on the well to fill the surface of the well with cement. We believe the 5.5 inch diameter casing was cemented at one time inside of the 7 inch diameter casing thats why we could not get any on the 5.5 inch diameter casing from the well. |
