1894554221 2016-06-06 Final Restoration No Violations Mr. Theodore Hach, the landowner, plugged this well on 08/14/2006 through the Orphan Well Landowner Grant Program. A plug report is on file in the Division database. Todays inspection was conducted to verify final restoration requirements have been met.

My inspection found an approximate 12 high, 2.45 diameter steel vertical line with a steel hood, that is open on both sides, coming from the ground in the area of the plugged Orphan well. The plugging report mentions a vent was placed on the well after plugging operations were completed, due to a 2-3 bubble gas leak every 10-15 minutes. The vent is too high to check for the presence of gas. My Sperian gas meter showed 0% LELand 0 ppm H2S at ground level at the base of the vent. There are not evident areas of dead grass or cracked soil in the area of the plugged well. A rusted plug is in the side of the pipe to monitor gas readings. The plug has rusted shut and cannot be removed without the proper tools. A house was not present on the property when the well was plugged. A house was built on the property in 2009 and is approximately 24 east of the well. A septic system is evident approximately 15 northeast of the plugged well. The laterals travel away from the well. The Ashtabula County Auditors site shows the property is currently owned by Gloria Daniels. I spoke with Gloria Daniels and gave her the Division contact phone numbers for future reference. She was aware of the plugged well when she purchased the property. Municipal water is utilized for this residence. Reviewed the inspection findings with Supervisor Chini. Final Restoration is passed.
