My inspection found the valve on the flow line from the wellhead to the separator closed. The domestic tap at the wellhead was open at this time. The production chart indicated pressure at the separator and the inflow and outflow valves are open. The stenciled ID sign on the 100 barrel tank has been updated with the required information and satisfies Chapter 1501:9-9-05(A) of the Ohio Administrative Code. An inspection of the affected area on the southwest side of the 100 barrel tank found that none of the remedial activities required by the prior Compliance Notice have been undertaken to address the pollution and contamination issues within the tank battery. Big Sky Energy Incorporated continues to be in violation of Chapter 1501:9-1-07 of the Ohio Administrative Code and Chapter 1509.22(A) of the Ohio Revised Code. Note: I spoke with the landowner during my inspection and he informed me that his house gas was shut off without any notification and that he was responsible for turning it back on. He also stated that the operator has made no royalty interest payments to him in several years. |

1462196767 | 2016-04-18 | Production Wells | Violations Noted | I conducted this inspection to follow---105 requiring Big Sky Energy Incorporated to suspend all oil and gas operations in the State of Ohio. My inspection found the wellhead equipped with a 1.5 plunger lift system. The gate valve and tubing flow line valve were open and a domestic tap was on and charged at 18psi. The well is producing to a horizontal separator and the flow line and sales line valves are open. The 31 day production chart in the separator was changed on 2-29-2016 and is currently recording production and over printing the previous month. The plunger lift timer is running and set to cycle in three (3) hours. A gauge at the separator indicates 68psi and I can hear gas moving through the pressure vessel. The well produces to a diked tank battery containing one (1) 100 barrel production tank and one (1) 50 barrel brine tank. The ID on the tank is insufficient as the well name, well number, and emergency contact number are worn off and not legible. The dump line from the separator to the 100 barrel tank has no integrity where it attaches to the top of the tank and has leaked/overflowed 8-10 gallons of crude oil onto the ground inside the tank battery. The oil has pooled in a 3 X 3 area at the base of the tank on the southwest side and is required to be cleaned up within one (1) week of the issuance of this notice. Big Sky Energy Incorporated has failed to comply with the above orders to suspend oil and gas operations in the State of Ohio and shall immediately cease the production operations of this well as required by the temporary restraining order issued by the Trumbull County Court of Common Pleas, dated 4/11/2016. |
