1717002861 2018-09-17 Production Wells Violations Noted The tank battery consisted of 2-100 barrel steel tanks, an approximate 80 barrel tank a vertical separator and a plastic 40 barrel tank that was full and contained brine. The plastic tank had a crack in it about 6 inches above the 2 inch fitting. There was crystalized chlorides on each side of the crack and a small brine scald below the tank. The tank will need emptied and removed from the location. The affected area will need remediated. There was only partial identification found at the tank battery. There was no contact number. I stopped by Rogers house to speak with his daughter who has previously stated she is working on getting stuff transferred into her name. I left one of my business cards in the door after no one answered.
1871269794 2013-11-27 Production Wells Violations Noted The landowner asked to meet me at the well to discuss concerns of his. During our meeting on 11/22/13 while we were discussing well 34.009.2.1546 the landowner showed me an area of contamination on the property. After further inspection it was determined the contamination had originated from a 2 steel line running from well 34.009.2.1558 to the tank battery. An area located at 39.34213 -81.95199 was found to have soils contaminated with crude oil and brine. The area was roughly 45 feet long and 15 feet wide in places. The leak along the line has been repaired in the past and straw had been laid down over the contaminated area. Any contaminated soils along the line shall be removed and disposed of properly. Any affected areas shall have soil replaced, seeded and mulched, and steps shall be taken to establish vegetative cover.
1906866918 2007-07-02 Production Wells Violations Noted On 7/2/2007, Dennis Deavers with the OEPA contacted the Division of Mineral Resources Management at 9:15 am. Mr. Deavers stated that a flow line broke resulting in oil being discharged into a pond. He states that Ms. Debra Lucas would like to be informed of the inspection results, 614-728-3858.

An inspection conducted on 7/2/2007 revealed that approximately 1 barrel of oil was released from a plastic production line into the pond. The production line was located above ground on a steep embankment. Mr. Roger Stright, owner of the Lucas #2 well, was repairing the broken production line at the time of the inspection. Mr. Stright stated that the pressure dropped 2 weeks prior to the oil being released into pond. Mr. Stright also stated that at least 2-4 barrel could eventually drain into the pond. Mr. Stright displayed a section of pipe that appeared to have been shot by a .22 caliber rifle. Mr. Stright was instructed to purchase booms to contain spill near the source along with absorbent pads to remove oil. Additionally, he was informed that the contaminated soil along the banks of the pond and around source of the spill would need to be removed and taken to a landfill. No fish kill observed at the time of the inspection. This case will be handed over to the Lead Inspector, Steve Ochs.
