0367480061 2013-07-15 Production Wells Violations Noted Upon inspection I found the wellhead shut in. There is a full 100 bbl tank adjacent to the wellhead. There is no dike around the tank and there is no identification posted at location. Operator must post proper identification at well head and construct dike in order to meet SPCC requirements. Dick Poling is sending in for a permit to plug this well in the next few days and plans on plugging the well sometime between now and fall.

0872906098 2011-06-30 Preliminary Restoration No Violations Inspection this day found deep Beekmantown well shut in. Tank nearby is not tied in, well not producing. Reclamation has been started - weather has been too wet to finish until now. Grass still need sown and mulched to pass.
1375886331 2013-08-07 Plug / Plug Back No Violations Wright well services continued to try and rip pipe and had several blades break. Kenny then left to return the blades. He will call me when he returns to location. There was noone on location when I returned around 315pm. I called Dick and he said Kenny would have to be back today or tomorrow to rip the pipe.
1375987675 2013-08-08 Plug / Plug Back No Violations Appalachian Wireline services came out to the location today and shot the 5.5 at 3172 however they could not get the casing out of the hole. Randy Wright spoke with Dan Goins who informed him that he could perf and squeeze the 200 lime plug, so they shot the casing for the plug at 2908-2912. Randy is currently running tubing back into the hole.
1376062967 2013-08-09 Plug / Plug Back No Violations I arrived on location for the Big Lime plug job. The bottom of the tubing was placed ~ 50 below the perf at 2957. Formation Cement set a 200 plug from 2757-2957 using 58 sacks of Class A cement. Kenny was going to pull tubing out of hole and had Appalachian Well services line up for 1:00pm to shoot off the 5.5. The plan is to pull all of the 5.5 and run back in and spot the rest of the plugs on Monday.
1377095385 2013-08-21 Plug / Plug Back No Violations I was on location to witness formation set the Clinton plug from 3864-4064. They pumped 25 bbls ahead to make sure the Packer Shell would take the water. Formation set 200 plug through the 5.5 casing displacing the cement with 92bbls of brine. Kenny ripped the pipe at 2983 and pulled two joints up to 2923. We will set a 300 big lime/rip plug tomorrow from 2623-2923 through the 5.5.

1377183507 2013-08-22 Plug / Plug Back No Violations I was on location accompanied by Inspector Doug Bennet. The plan was to set Big Lime/Rip Plug followed by the surface casing and surface plug. The 5.5 casing was locked up down the hole most likely due to the swelling/caving in of the Ohio Shale. The new plan was to try and go up a few joints in order to rip pipe and set the plug. When I left the were planning on rigging up the jars.
1377632224 2013-08-27 Plug / Plug Back No Violations I was on location to witness Formation Cement set a 200 Big Lime plug from 2630-3030, and a 200 surface casing plug from 1292-1492 through 3.5 casing. They then set a 200 brush and stone plug followed by a surface plug. The surface plug dropped down and fluid level was around 400. Another brush and stone plug was set at ~125 in order to avoid fluid/cement loss around 200 followed by a good cement plug to the surface. The surface plug remained at the top of the hole.

2144094465 2013-08-14 Plug / Plug Back No Violations I was on location with Wright Well Services and Formation Cement in order to set a 400 bottom hole plug. A 574 plug was set from 5838-6238 with 25 sacks of Class A cement. The plug was displaced with 53 bbls of brine. 8/15/2013: Appalachian Well Surveys was on location and identified the top of bottom hole plug at 5630 and the top of cement on the 3.5 at 5465. They shot the casing at 5412 and could not pull it loose from the hole. There was at least 2 of stretch. Appalachian went back in and shot at 5371 and the casing came loose after shooting it again at 5374.
