0332010116 | 2014-08-01 | Production Wells | No Violations | I was on location for a status check inspection. I found the well not in production. The well was equipment with a pumping unit. Two, ball valves on the flow line off of the tubing were closed. The tank battery consisted of two, 100 barrel production tanks, two, 50 barrel poly brine tanks and two separators. A dike surrounded the production equipment. An identification sign was posted. A plug was installed on the load line. The lease road was in stable condition. The tank battery was shared with permit number 2-3934. |
1048414908 | 2019-03-25 | Production Wells | No Violations | I conducted a compliance notice follow-up inspection for compliance notice # 1268132571 and complaint #5923, regarding installation of identification and plugging the well. At the time of this inspection, the well did not have identification installed and the well has not been plugged. There has not been a plugging permit obtained from Styker Energy, at the time of this inspection. I attempted to contact Stryker Energy, LLC by calling multiple phone numbers associated with this company and all of the phone numbers have been disconnected. I was unable to connect with this company to address the open compliance issues. |
1268132571 | 2018-08-14 | Production Wells | Violations Noted | Follow-up to Non-Producing Well Complaint #5923. I spoke with the attorney for the landowner Michael Grove on 8/8/18 and told him I would inspect the wells the week of 8/13/18. My inspection today found this well equipped with 1.5 tubing and rods and a natural gas powered pumping unit. There is currently no motor to operate the pumping unit and there is a 30 barrel tank plumbed in in front of the wellhead. Small trees and tall weeds have engulfed the pumping unit to the point where it cannot be thoroughly inspected. The tubing and 4.5 production casing flow line valves are closed and the well is shut-in and not capable of production in its current state. This well produced to a diked tank battery to the west and is shared with the Stuard Unit 1 (PN#23934). The battery contains two (2) 100 barrel tanks, two (2) 50 barrel poly tanks, and two (2) horizontal separators and each well produces through a dedicated set of equipment. The poly tank for this well currently contains 40+ barrels fluid. The inlet and outlet valves on the separator are closed and the well is not in production. There is a domestic tap on the separator sales line and the supply valve is closed. The required identification is posted at the tank battery, but is not posted at the wellhead in violation of ORC 1501:9-9-05(A)10. Production records indicate that Stryker Energy, LLC produced the well for 84 days during the 2012 reporting period and produced 78 Mcf of natural gas, zero (0) barrels of oil, and zero (0) barrels of brine. The well has produced zero (0) Mcf of natural gas, zero (0) barrels of oil, and zero (0) barrels of brine for the last five (5) reporting periods, has not been placed in Temporary Inactive Status, and has not been approved for another activity by the Chief. This well is in violation of 1509.062(A) of the Ohio Revised Code and it is determined that the well is currently incapable of production. The well owner/operator shall submit a written plugging schedule to M |
1548429788 | 2019-01-31 | Production Wells | No Violations | I conducted a compliance notice follow-up inspection for compliance notice 1268132571 and complaint #5923, regarding installation of identification and plugging the well. At the time of this inspection, the well was equipped with a pumping unit and not in production. No identification has been installed at the well location and the well has not been plugged. There was a 30 barrel tank plumbed in front of the wellhead. Vegetative maintenance had not been conducted at the well location. The inlet and outlet valves on the separator were closed as well as the valve on the domestic line off the separator. The sales line and the supply valve was also closed at the tank battery location. Identification was posted at the tank battery, but still needs installed at the well location, due to a shared tank battery with well 2-3934. |

1902728926 | 2018-01-11 | Production Wells | No Violations | I conducted chiefs order follow-up inspection on this well, regarding liability insurance. The well was equipped with a pumpjack and the valves on the annulus and flow line were closed. The tank battery consisted of two 100 BBL production tanks, two 50 BBL poly brine tanks and two mechanical separators. Valves on the separators were closed. There was a domestic line at the tank battery with a meter, drip bottle and regulator. The valve on the 1 inch line was open. The tank battery was diked and is shared with well 2-3934. There was legible signage located at the tank battery but needed added at the well due to a shared tank battery. The lease acess was stable. I called Stryker Energy and spoke with manager Gary Regan. He indicated that the insurance had been sent in to DOGRM, Columbus office and was being processed. |
