0038027394 2014-09-18 Plug / Plug Back No Violations I was onsite today to witness the plugging of this well. Formation is setting the bottom plug today through the tubing and the Trenton plug. After the plugs were set Kenny Wright pulled out the tubing then set up to rip the 5.5. They were able to get the pipe ripped at 2,940ft. They will finish plugging the well tomorrow.

Dick is going to have Appalachian come in and perforate the well at 5155 to see if they can get any production before they decide to plug the well. Appalachian is scheduled for the 15th at 12pm.

They contacted me to let me know they would be getting a grout truck to come in and top the well off. We filled the well back to surface with cement. I returned the next day to make sure it stayed at the surface. I contacted Zack and told him he could cut the wellhead off.

I was unable to write the well off because the pumping unit and production tanks were still on location. I looked down the well and the cement has settled so I contacted Mr. Poling to let him know about the situation. He is going to set up a grout truck to come in and top off the well. He will contact me when he has the truck set up.
