1136313877 2006-01-03 Production Wells No Violations Chk on the report of an oil spill. The spill is mostly cleaned up a crew is on the site removing contaminated soil from around the dike area. The road ditch has been dipped out to remove contaminatied soil.
1186725415 2007-08-10 Production Wells No Violations On site by request of Garry VanDyke (pumper). Approx 12:30 AM the neighbor noticed the odor of oil and called Garry. He went to the site and found that the flow line had ruptured and let an undetermined amount of oil out on to the ground. The oil went to the road ditch and then south approx 50 foot into a culvert and under Co Rd 24, a dam was installed at that point by Mr.Howerd (neighbor). I was notified at approx 1:00 AM. Arrived on the site at 1:22 AM, a vac truck was on site and clean up had been started. The bulk of the free oil was removed w/the vac truck and the shut down untill day light to further asses the situation for further clean up.

The well has been shut in upon observation and until the nipple can be replaced. An estimated eight barrels of brine water were released and flowed down grade behind the tank battery and to the public road. An elongated area less than 3000 square feet were impacted. Two earthen dams were created to prevent continued flow. Lime dust will be spread of the area for remediation.

1512059832 2017-11-30 Production Wells Violations Noted Received notification from the operator of a release from a pinhole that developed in the pumping tee in the actively pumping Bible #1 well. Production brine with a small amount of hydrocarbon sprayed on the ground and traveled east down the gravel lease road (following a drainage rut) 225 feet to County Road 24, turned south and pooled along the road berm a total of 355 feet south. At approximately 116 feet on CR 24 the release entered a road culvert and crossed CR 24 East into a drainage tributary toward Alum Creek. The well was shut-in and remediation began with Fishburn Services flushing the leases road with 1.5 loads of fresh water and Mullins Energy Services vac truck working at the entrance off CR 24. Will continue to monitor remediation in conjunction with Emergency Response Coordinator Ms. Vikki Bunting.
