0335410958 2018-08-22 Production Wells No Violations On the day of my inspection I witnessed that the well was no longer in production. Portions of the well head had been removed and the well was no longer connected to production equipment. EQT Drilling Engineer Steve Bishop informed me that they had installed a back-pressure valve in the tubing hanger and Valve Removal plugs in the well head on 8/3/2018. The well heads were contained within a steel cage. Annular pressure monitoring was still in place and the 9.625-inch diameter casing gauge read 235 pounds per square inch, the 13.375-inch diameter casing gauge read 460 pounds per square inch. Some of the mobile fluid storage tanks had been moved onto location and EQT was currently waiting on the plastic liners for secondary containment before moving more equipment onto location.
0536194750 2014-09-08 Casing String Witnessed No Violations Falcon 39 drilled a 23 1/8x380 open hole and ran 18 5/8x361 casing (87.5 #/ft). Casing was centralized with 5 centralizers. Baker Hughes was onsite with 410 sks of class A (15.6 ppg, 1.21 yield) cement to complete the cement job. 50% excess was ran for the open hole porportion. Previous casing is a 24x236 mine string. Good cement returns to surface.

0704307428 2014-08-27 Drill / Deepen / Reopen No Violations On the day of my inspection H.A.D drilled the open hole for the mine string but the shoe used did not allow enough annular space for adequeste cement. A bigger shoe was ran in and reamed to bottom. No violations noted.
0865678795 2014-09-25 Drill / Deepen / Reopen No Violations Falcon 38 is drilling the 12.5 open hole one fluid at a depth of 6600. TD of this section is 8400. Rig had up to 163ppm H2S in the flare lines when blowing out the hole but numbers fluctuated between 0 and 1ppm while drilling. H2S was encoutnered around 5400, gas around 5100, and rig switched over to fluid at 6476
0989392660 2014-08-28 Casing String Witnessed No Violations HAD Drilling on site and they drilled a 25 hole and then they ran 24 casing set at 240 and Baker Hughes cemented pipe in place with 127 sacks of cement and they had good returns throughout the job.

Note: Coal seam was noted at 185-190.

1194535960 2018-02-08 Production Wells No Violations On the day of my inspection I witnessed the well was currently shut in and had undergone simultaneous operations preventions. At the time of my inspection I witnessed that all valves were shut and had lock out tag out preventions at the well head. There were transducers being used to monitor annular pressures on the 9.625-inch diameter deep intermediate casing which was currently at 440 pounds per square inch. The 13.375-inch diameter intermediate casing pressure was at 440 pounds per square inch. After speaking with EQT completions engineer Andrew Sorg he informed me that the producing wells were being monitored by the EQT production operations department. I informed Carlos Leal the onsite company representative that I witnessed annular pressure during the drilling process and that they needed to closely monitor the annular pressures on the producing wells.

1384539097 2018-06-13 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing. Annular pressure monitoring was in place for the intermediate casing gauge read 561 pounds per square inch, the surface casing gauge read 563 pounds per square inch. Production equipment included an individual Gas Processing unit. Shared production equipment included a dehydrator, 2 phase separator, contact tower, and 3- 400 barrel purge tanks shared with 34013208180000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.

1723668621 2019-04-08 Production Wells No Violations Status check inspection found 2 wells on location that have been drilled completed and produced. They are identified as the #2-H and #4-H. These well are temporally shut in with the production flow lines disconnected during the drilling and completion process of 3 additional wells on the pad identified as the # 1-#3 &#5 wells. There is a Rice Energy Mid stream operation on the pad that is currently idle. Also present at the rear of the pad is a above ground storage tank (AST) that is being used for water to stimulate the Razin Kane well pad that sits in close proximity to this pad.

The well appears to be free flowing without any artificial lift technology present at this time.
