0269403712 2008-12-19 Administrative Inspection No Violations At 12:47 pm, I called Mr. Doug Adams on his cell phone, 304-588-3333, and left a message stating that I have not received a schedule for the Sanners & Brown #1, Permit no. 357 and the Conley #2, Permit 315. He was instructed to call me back.

Mr. Adams called me back at 3:02 PM. Mr. Adams states that he is going to have Mr. Tommy McDonald with PetroQuest send me a letter. I informed Mr. Adams that I would like to have a schedule so that I could compile this information together before we met with Mr. Rick Simmers. He was informed that the best way to avoid enforcement would be to do a Consent Agreement and this schedule would be built into this contract. Mr. Adams states that they have maps and easements all together. He states that he will have Mr. Mcdonald give me a call

A call from 440-368-3558 was missed on my cell phone at 3:59 PM. This call was returned at 4:01 PM. Ms. Miriam Faudree, a secretary with PetroQuest Resources (parent company)/ Mountaineer Gas Transmission states that she was contacting me for Mr. Tommy Mcdonald. Ms. Faudree states that she is sending me an email of a letter that had been sent on Wednesday- Not received at this time, but could be at the Jackson Office. Further stating that I will receive email within 5 minutes.

Mr. Doug Adams, the owner, has met with Joe Hoerst regarding the idle wells. Mr. Adams presented a plan, which showed that all idle wells would be placed back into production tied into new gas line for commercial sale. During the inspection, it was noted that the well was still idle.

She wanted to know status on PetroQuest, Mountaineer Energy, Mr. Tommy McDonald and his partner Mr. Tajh Williams. She was informed that I have not heard anything. They will be drilling possibly 20 wells for them and would also like to do some themselves (Logan Gas). She was informed that I have had no correspondence with Mr. Williams. My correspondence has been mostly with Adams Oil, Mr. Doug Adams. She referenced Dougs son, Mr. Jamie Adams with Majestic Oil.

Asked if they have made any progress with pipeline? She states that they had a meeting with PUCO in regards to Jurisdiction and Non-

Mr. Adams had previously stated, in a telephone conversation, that a crew was scheduled for January 5th to perform well head installation. No activity was observed at time of the inspection. Identification has now been placed at the well, but is missing a lease name and does not show current well owner, Adams Oil and Gas. The identification is showing Mountaineer Gas Transmission.
1244744427 2008-10-15 Production Wells No Violations A follow-up inspection revealed non-compliance. The well is still idle and not capable of production. Identification has not been provided. No action has been taken to resolve the previously noted violations. During the inspection, it was noted that the above ground casing is damaged. A section of this casing was found to be peeled around the side. Glass and dirt were observed on the ground surface located around the well bore. The well does not contain rods or tubing. No flowline or tanks were found on the property. A gas odor was also noted at the well location. The landowner, Mr. Dallas Vance, was informed of the inspection results. Mr. Vance states that he removed the bent flowline that had been placed at the well to serve as a marker. The well owner, Adams Oil & Gas will be contacted by telephone regarding this well. New photographs were obtained from this inspection.

Returned this call at 11:02 AM. Mr. Adams states that he tried to contact Kilbarger today, but did not have any luck. He wanted to know if I received a letter from Mr. Tommy Mcdonald. He was informed that I received the letter via email. Mr. Adams states that the last he heard was that Kilbarger was working on a right-of-way. I informed Mr. Adams that I had spoke with Kilbarger and that they were working on trying to obtain a line and would need to lay line- I mentioned that it could be a right-of-way too, but Kilbarger had explained it to me in a way that they were working on another line. Mr. Adams states that he has a book with his right-of-ways and maps. He was informed that we would need this information for our meeting. He states that he could meet next Wednesday. Experiencing bad connection and will call him back later today. Called Mr. Doug Adams on his cell phone, 304-588-3333, at 1:55 PM. He was asked how long it would take to address Sanners & Brown, Permit No. 357? He states that we need well heads on both wells ( Conley and Sanners & Brown) and they need contained. He has a geological report that he will review the first of next week. Mr. Adams states that he has work the scheduled for 1/5/2009- will dig around Sanners & Brown casing with hoe and install a well head. The same work is scheduled for the Conley- a swab rig will be placed on this well to see what fluids they have. Fluid had been down 4.5 seconds. Mr. Adams states that he looked at all the Red Gas wells. His son was in charge of the Blauser package wells. Mr. Adams states that with the Pike Petroleum package he has not been to half of these wells. He further states that if there are any environmental problems to let him know.
