A chloride check of the surface water around each sump was done. Annular pressure monitoring was in place on both the 9.625-inch diameter casing gauge read 1245 pounds per square inch, the 13.375-inch diameter casing gauge read 400 pounds per square inch. Production equipment included an individual Gas Processing unit with 2 additional sand traps, and 4- 400-barrel production fluid tanks shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There was also an additional 500-barrel portable fluid storage tank on location that was not hooked up to any other equipment and did not have secondary containment around it. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current. |

The sediment ponds had some of the contaminated soil removed by surper sucker but they were unable to suck it out from with in the cattails. OEPA has stated that the sediment ponds are NOT wetlands by their definition and can be remediated and reclaimed. The removal of the pads is nearly complete but there are now many more holes in the contailment. A soil survey will be done when the containment is removed. This will hopefully pinpoint the area of the original KCl spill so that it can be remediated. |

1487850123 | 2017-02-22 | Drill / Deepen / Reopen | Violations Noted | One location to meet with Denny Furmanek, Rice Energy Environmental person about a spill of Potassium Chloride (KCl) water of unknown volume. The drilling rig has moved off the location and workers are removing the large interlocking plastic pads that sit on the containment. The KCl water came out from under the containment and got into a sump on the SW side of the pad and migrated to the sediment ponds also on the W side of the pad. Maximum chlorides found were 1200 ppm in the upper sediment pond 415 ppm in the lower pond. From the ponds it went into a farmers pasture in a tri-angular pattern 75 wide at the top and 215 to the point. The contamination is approximately 2 deep in the soil. The sediment ponds have been emptied and both sumps have been closed. The area where the KCl water is coming out from under the containment has not yet been remediated due to the fact that there is a rain event coming and the workers have not yet completed removing the contaiment (3-4 days out) and more contaminated water could come out from under the containment during the rain event. They are not yet certain of the source of the KCl water or how it got thru the containment. Once the larege interlocking pads are removed an accessment can be made. Workers will remove a fence to access the farmers field and fence off the contaminated area so that it can be remediated along with the sediment ponds. A spring some 300 W from the pad has not been affected. No more KCl should get off the pad or out of the sediment ponds. |

The sales line pressure at the production tree is 1,153 pounds pressure. There is annular pressure monitoring present with 45 pounds pressure on the 13.375 diameter casing well head section and 122 pounds pressure on the 9.625 diameter well head section. The production fluid facility consists of 4 steel tanks capable of holding 400 barrels fluid volume each, the tanks are situated inside a polymer lined steel containment dike area. Also present on location is 10 temporary 500 barrel steel tanks that are situated inside a polymer liner, the tanks are empty and not currently in service. Well identification is present along with hazard warning and emergency shut down devices at all critical points on the well pad. There is a previous violation #1487850123 issued on 02/22/2017 that I am referring to my supervisors for closure. |
