0142276242 | 2017-06-13 | Drill / Deepen / Reopen | No Violations | Sidewinder 103 was still attempting retrieval of the lost bit. They were able to move the bit up hole 10 feet when the jars failed on them. The pull out of the hole and install new jars at which point they will continue fishing operations. |

0969425818 | 2017-06-21 | Drill / Deepen / Reopen | No Violations | During this inspection Sidewinder Rig 103 was tripping out of the hole because there was a motor failure down hole while drilling their 12.25 inch diameter open hole section. The current depth is 7200 feet. The rig will change out the failed tools and continue drilling this section of the well bore. |
1004128520 | 2018-02-14 | Completion Testing | No Violations | This well is equipped with 5.5-inch diameter production casing. The well is currently shut in as no completion operations have commenced. There are mats currently stacked out on the side of the location. There is midstream equipment that has been installed and is awaiting completion testing following which, production operation will be tied in. |

1345213564 | 2019-01-15 | Production Wells | No Violations | Status check inspection found (3) wells total on this well pad with all 3 wells in production. All 3 wells are producing through a production tree through 3 manual gate valves and one emergency shut down valve with an electric choke present in line. The said well does have 2 diameter riser pipes present attached to the well head A&B sections outlet ports. The riser pipes are equipped with valves and gauges (both electronic transducers and manual gauges) the purpose of the gauges is to monitor annular pressure. The A section well head pressure is zero (PSI) pounds per square inch, the B section pressure is 900 psi. The 5.5 diameter production casing pressure is 2,200 psi and the flowline pressure down stream from the choke is 1,600 psi. There is an emergency shut down control point at each of the 3 well head along with each critical point on the well pad. On site production equipment consists of (3) gas processing units along with a tank battery that contains (4) steel tanks capable of holding 400 barrels of fluid each along with a steel 50 barrel tank, all situated inside a polymer lined steel containment dike area. There is a third party (EQT) midstream pipeline station adjacent to the right front corner of the well pad. The lease road and well pad surface is stone construction and in good repair. The well pad per miter is protected by an earthen berm. The Preliminary Restoration is due however there is snow on the ground during my inspection. |

1497455886 | 2017-06-14 | Drill / Deepen / Reopen | No Violations | Sidewinder 103, with Baker Hughes assistance, has successfully recovered the 17.5 inch diameter bit that was previously stuck in the hole. Drilling operations shall now resume. |
1509839535 | 2017-06-08 | Drill / Deepen / Reopen | No Violations | Sidewinder Rig 103 is drilling the 24 inch diameter mine string portion of this well bore. Once they reach their total depth within this well bore they will install and cement the 18.625 inch diameter casing string. |

1987757893 | 2017-06-29 | Casing String Witnessed | No Violations | Sidewinder Rig 103 had reached total depth of the 24 inch diameter open hole section of the well bore at a depth of 648 feet. The 18.625 inch diameter casing was then ran into the open hole. Halliburton cementing was on site and performed the cementing operations. Inspector Bill Stolarik was also on site to witness this casing installation. Please see his report for further details. |
