1098172144 | 2018-11-14 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. All erosion control filter sock material placed on the outward slopes during construction have been removed. The well pad and lease road are stone construction and in good repair. Signage is posted at the lease road entry point and at each well head with hazard warning labels on the tank battery. Emergency shut down devices and fire extinguishers are situated at each critical area of the well pad. There is a 3rd party mid stream operation along the lease road on the left side entering the well pad. The lease road does have a gate to restrict access. A total of 6 wells are present on the pad with the said well equipped with an A&B section well head along with a tubing head production tree. 2 diameter riser pipes with valves and gauges are present and attached to the well head A&B sections for the purpose of annular pressure monitoring. The A section gauge which is monitoring the 13.375 casing pressure reads zero pounds per square inch pressure (psi) the B section gauge which monitors the 9.625 diameter casing pressure reads 800 pounds pressure. The 5.5 diameter casing pressure is 1,700 pounds pressure and the tubing head pressure before the choke valve is 1,100 pounds pressure. The production tubing well head valves are open to the sales line and an emergency shut down device is situated on the production well head tree. There are (6) gas processing units present on location and on the rear left hand corner of the well pad is the production fluid facility that consists of (4) steel construction 400 barrel fluid volume tanks along with a 50 barrel steel tank. All situated inside a polymer lined steel containment dike area. |

The rig has drilled a 24 diameter well bore through the affected mine rubble zone noted to be between the depths of 494 to 505 with the measurements from a 28.5 drilling rig Kelly bushing heigth above grade level. The total depth drilled was 614. Upon reaching total depth the well bore was circulated on with air for a period of time before the company administered a fresh water gel sweep in attempt to pre condition the hole before pulling the drill string. Immediately upon administering the fluid to the well bore the drill string became stuck and the rig has been unable to pull the drill string up past 526. The company will continue to work the string and attempt to pull free from the debris. They have drill string rotation. |

Pioneer 71 drilled a 24 inch diameter open hole to a depth of 614 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming Casing ran 614 feet of 18.625 inch diameter casing, j-55, 87.5 lbs/foot. Casing was centralizer with 3 centralizers. Shoe track is 34 feet long. Cement Baskets were set at 358 and 418 feet. Mine encountered at 495-505 feet. Packre set at 377 feet. O-tex was onsite with 900 sacks of class A cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.21. Cement displaced with 165 barrels of fresh water. 50 barrels of gel pumped ahead of cement. No circulation returned to surface. I had to leave job after dropping the plug as I ran out of hours. I did not witness cement pumped down backside to set up on packer. Mix water has pH of 7, 271 ppm chlorides and 85 degrees Fahrenheit. |
