The well head A & B Sections are Cactus with the completion stack being from B&B. The well head B section which is the 5.5 x 9.625 annular space does have pressure control attached with electronic transducers in place to monitor pressure. Transducers are also attached to the A section which monitors between the 9.625 and 13.375 well head section. The well pad surface is of stone construction with earthen berms in place. |

1506981293 | 2017-10-02 | Casing String Witnessed | No Violations | On site inspection accompanied by ODNR Inspector Andrew Corder with H&P Drilling rig 608 on location along with Wyoming Casing Service and Stingray Cement Company. The drilling rig has drilled a 24 diameter surface borehole to a measured depth of 466 off of a drilling rig Kelly bushing heigth of 26 above ground level with a loss of weight on bit while drilling at 407 to 418 indicating the top and bottom of a mine void. The rig pulled the drill string and inserted 466 of 18.625 diameter 87.5 pound per foot J-55 grade buttress thread casing with a guide shoe and float collar casing equipment ran along with a Weatherford swell packer set in the string between the depths of 383 and 356. The casing went to the bottom of the hole without incident and was cemented by Stingray in a 2 stage process with a 35 barrel bottom cement slurry pumped consisting of 166 sacks of class A cement with .03% calcium chloride and .25 pound per sack cello seal. The cement weight was verified at 15.6 pound per gallon by weight of cement and the cement was displaced to the float collar with a total of 131 barrels of fresh water. Upon the landing of the cement plug at 625 pound per square inch (psi) the pressure was increased in 200 psi increments until the packer inflated at 1,217 psi. Once the packer was confirmed as set the casing was pressure tested to 1,200 psi. The company then attempted to top the annular space off from the back side with 45 barrel class A mix with an annular capacity of 85 barrels. The company was then required to run a 1 tubular string down the backside of the 18.625 annular space above the packer was then topped off with a total of 82 barrels of class A cement weighing 15.6 pound per gallon by weight of scale. We were on location from 11:00 am until 22:30 pm. |

The well head configuration consists of a Cactus Company lower A section that is attached to the 13.375 diameter casing string, this is a new style well head system that the 9.625 and 5.5 diameter casing string land out in the A section by the means of mandrill hanger systems. The B section well head assembly consists of the 5.5 production casing flow cross only. The company has removed the lower and upper completion master valves and installed a service cap and 2 butterfly style valves to allow monitoring and pressure relief of the internal of the 5.5 diameter production casing which has not had the toe sleeves open or the initial set of perforations shot in the casing. A company identified as Full Circle is on site installing riser pipes on the well head A sections to monitor annular pressures. 7 wells are present on location and no production equipment is present on location. A mid stream pipeline is being installed to the pad midstream service point. 7 wells are present and have been drilled on the pad. The rig that drilled the wells identified as H&P 628 is disassembled and stacked out on location. |
