0156125512 2017-12-11 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 1615 feet. Previous casing set and cemented was 18.625 inch diameter mine string casing to a depth of 297 feet. Wyoming Casing ran 1609 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot. Shoe track is 44 feet long. O-tex was onsite with 1360 sacks of class H cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. After the plug was dropped the pump truck broke down and was unable to cement to well to surface.
1098747021 2017-12-07 Drill / Deepen / Reopen No Violations Pad is located at 72601 Henderson Ridge Road, St. Clairsville, Ohio 43950. On the day of my inspection Falcon 26 is moving equipment onto location. 3 conductors were set earlier this week. Company Man Tony Shepard believes they will spud in on Friday evening.
1512485023 2017-12-05 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex Pumping LLC pumped 330 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 1.5 percent Calcium Chloride and 0.25 pound per sack cello-flake. O-Tex then displaced with 71 barrels of water, leaving 15 feet of cement in the casing. Good cement returned to surface and remained static. Approximately 11 barrels of cement returned to surface, Ascent left approximately 18 inches of cement in the cellar as a seal.
1513180037 2017-12-14 Casing String Witnessed No Violations On the day of my inspection I witnessed falcon 26 run 300 feet of 1 inch tubing down the backside to top out the cement job from Monday 12/11. One of the engines on pump truck would not start. Called another truck that was on Dreamweaver pad and it arrived 2 hours later than expected. O-tex onsite with 800 sacks of class H neat cement. Bond log shows cement top around 700 feet. Pumped 100 barrels of cement, 10 barrels returned to surface at 14.6 ppg.
