0124224501 2018-01-02 UrbanProduction Wells No Violations Follow-up to Chiefs Order 2017-441 to determine if Stephen J Svetlak, LLC has suspended all oil and gas operations in the State of Ohio as required by the Order. I was on location with Northeast Regional Supervisor Tom Hill to conduct the follow-up inspections. The inspection today found that the valve on the sales line has been closed and the chart indicates that the well is not currently in production. This well is in compliance with Chiefs Order 2017-441.
0165382326 2017-05-17 Production Wells No Violations I conducted a Chiefs Order follow-up inspection on this well for Chiefs Order #2013-136 on 9/25/2013. The operator has failed to maintain proof of liability insurance as required by ORC 1509.07(A). The well is in production which is in violation of the Chiefs Order. The wells master valve is open and the valve going to the meter shack is open. The chart in the meter box is dated 5-9-17 and the meter was calibrated on 06-02-2016 by Gas Analytical Services. The diked tank battery consist of a 100 barrel tank. The well is not legibly identified..
0215239826 2018-02-12 UrbanProduction Wells No Violations Follow-up to Chiefs Order #2017-441. My inspection found the valve on the two (2) inch sales line closed before the sales point tie-in. The well is shut-in as required by the Chiefs Order until the required liability insurance is obtained and submitted.
0677662476 2013-10-15 UrbanProduction Wells No Violations I inspected this well to follow up on Chiefs Order #2013-136 issued on 9/25/2013. The operator, Stephen J. Svetlak, LLC, has failed to maintain proof of liability insurance as required by ORC 1509.07(A) and has been found in material and substantial violation for failing to maintain coverage. My inspection found that the well was still in production and is in direct violation of the Chiefs Order to immediately suspend all oil and gas operations in the State of Ohio and 1509.07[C] of the Ohio Revised Code. I documented that the well is producing into a Dominion/East Ohio Gas line and took pictures inside the meter run showing production and that the chart was changed out by Tri State Measurement on 10/01/13. The operator has not reported production since 2009. The ID sign on the tank does not meet urban production well requirements and has the wrong permit number on the ID sign (#0486 and should be #0427). This well is a swab well and produces to a 100 barrel tank that is inside a diked area. I could not locate a separator and the west side of the diked area was overgrown with 10-12 foot tall fragmites.

I was on location with Northeast Regional Supervisor Tom Hill and Mahoning County Inspector John Fleming to conduct the follow-up inspections. The inspection today revealed that the inlet and outlet valves at the meter run (sales point) remain open. The chart has continued to record gas production from the wellhead since my initial follow-up inspection on 12/22/2017 (see photos). The well remains in production and is not in compliance with Chiefs Order 2017-441.

Svetlak, LLC is in compliance with the order to suspend all oil and gas operations in the State of Ohio. My inspection today found the master valve on the wellhead open. I was unable to determine if the wing valves on the flow line were closed at this time. The valve on the one (1) inch line on the back of the wellhead is closed. A 100 barrel tank is located within secondary containment and there is no separator present. The valves at the meter run (MOA) are open and the production chart indicates that well is currently in production in violation of Chiefs Order 2017-441. The required identification is tattered and illegible.
