1342625578 2012-07-18 Plug / Plug Back No Violations Petro Evaluation is plugging the Cochran #1 (#3855) off a valid plugging permit. Five plugs have been designed and are being set by Omega cementing with Chipcos service rig. The work is being witnessed/run by Petro Evaluations representative Steve Franks. The first plug (bottom hole plug) 3600-3350 was set through tubing at 10:30-10:45 am. Chipco pulled tubing and went back in hole to rip 4.5 casing. Shut down for severe weather at 3:00 p.m. Will rip and finish plugging 7/19/2012.
1342725287 2012-07-19 Plug / Plug Back No Violations This is a summary of events which occurred while plugging to abandon the Cochran #1 (#3855) Trempealeau well in Gilead Township, Morrow County. Operator: Petro Evaluation, company representative Steve Franks, (330) 465-8866. Service Rig: Chipco, toll pusher John Fannin (Little Bit), operator Justin Mesaros. Cement Contactor: Omega . Water truck: S. & W Hauling. Plugging began on Wednesday July 18th with the bottom hole plug which was set through tubing and observed by myself and Jeff Frye. Chipco pulled tubing and shut down at 3:00 pm due to severe weather conditions.

7/19/2012 1) 8:00 am. Chipco go-in-- 2) 9:15 am. Hook-up Omega. Achieved circulation. Omega pumped the bottom of the Big Lime plug away at 9:30. At the start of circulation the rig personnel were getting low readings on their H2S detectors (my detector never registered any H2S during the event: it was on and fully charged). 3) 9:40 am. I walked back to my truck to get my phone (left my detector with the cement crew), at that time a large amount of H2S gas circulated out-to- 4) 9:50 am. We discussed what to do. John Fannin had an air tank and a fan for circulation which was not ready to go (hook-up to a generator). Given the uncertainty of their equipment and not knowing the air condition at the well head (given the weather conditions), we discussed calling EMS. Steve Franks agreed and asked that I call. 5) 10:05 am.

1347286272 2012-09-10 Final Restoration No Violations The final restoration is complete. Pits closed, debris removed and a landowner waiver received for access road and flow lines. The remaining storage is part of the Cochran #2 (#3876) tank battery.
