0304601317 2016-12-15 Production Wells No Violations On the day of my inspection with the ground snow covered, I witnessed that the well was equipped with a production tree and a lubricator for future use and valves were in open position. The production equipment included a sand trap; a line heater, 2-GP units and 2-400 barrel purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 2130 pounds per square inch on the tubing and 370 pounds per square inch on the casing. Annular pressure monitoring was present along with proper identification and production records being current.
0738895357 2016-04-19 Completion Testing No Violations On the day of my inspection I witnessed Deep Well Service Snubbing Unit was drilling out the last frac plug at 26285 feet. DWS will trip down to the toe before tripping out prior to tripping back in with production tubing. Production tubing packer will be set at 55 degrees in the curve at approximately 7500 feet with the tail of the tubing to be at approximately 8104 feet with flowback to follow.

Gauges present read 2840psi on the tubing and 1280psi on the casing. Annular pressure monitoring was present along with proper identification displayed. We completed a walk around with Eclipse Well Tender Bradley Paisley.

Gauges present read 3275psi on the tubing and 1670psi on the casing. Annular pressure monitoring was present along with proper identification displayed. Discussion was held on the readings with Eclipse Lease Operators Bradley Paisley and Danny Hreha.

Completed walk around with Eclipse Resources EHS Field Tech Mike Hartshorn revealing proper secondary containment was in place with minor tears. Grady Rentals was called and arrived prior to departure from location to make repairs.

Gauges present read 380 pounds per square inch on the tubing and 1450 pounds per square inch on the casing with annular pressure monitoring in place. Proper identification and production records were current. The Above Ground Storage Tank (AST) remains onsite along with 2-500 barrel empty frac tanks within containment.
1398487223 2016-05-09 Production Wells No Violations On the day of my inspection accompanied by ODNR Southeast Regional Supervisor Brent Bear, I witnessed flowback operations were completed with the well online. Completed walk around and met with Consultant Cristo Giannareas who stated the tubing was set on a packer with gas injection installation in place for future when the well loads up. Gauges present read 1700psi on the casing and 3350psi on tubing with valves open on the tubing and the product being sent to the Hayes CGF.
1452948165 2016-01-16 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Precision Rathole had drilled a 26 inch diameter hole to a depth of 120 feet and ran 120 feet of 20 inch 94#/ft. conductor pipe. During drilling water was hit at 18 feet leaving the wellbore full of freshwater. Universal Well Service was on site and gained circulation with 5bbls of freshwater, pumped 25bbls of gel, 5bbls of freshwater and cemented pipe with 260 sacks Type 1 cement with 3% CaCl and .25#/sk. of poly-flake with a weight of 15.6ppg and a yield of 1.2cuft/sk. Cement weight was verified by pressurized mud scales. Cement was displaced with 35bbls of fresh water and had good cement returns of 17bbls to surface.
1453938462 2016-01-27 Casing String Witnessed No Violations On the day of my inspection I witnessed that H&P Flex Rig #531 had drilled a 17.5 inch diameter hole to the depth of 1380 feet and hitting water at 1100 feet with a flow of 150bph. Wyoming Casing Crew ran1024 feet of 13.375 inch 61#/ft. J-55 casing and 327 feet of 13.375 inch 54.5#/ft. J-55 casing for a total of 1351 feet of 13.375 inch casing set at 3 feet above KB. KB height is 30 feet above ground level. Schlumberger was on site and gained circulation with 300bbls of fresh water, pumped 20bbls of gel, 10bbls of fresh water and cemented casing with1045 sacks of Class A cement 15.6ppg, 1.21cuft/sk. cement with 3% D154 Extender, .2% D046 Antifoam, .35% B348 FLAC, .2% Dispersant and .2% D013 Retarder. Cement was verified by mud scales and displaced with 199bbls of fresh water. Cement was circulated to surface with good returns of 40bbls, plug was landed at 630psi and pressured to 1205psi for 5 minutes and the floats held with 1bbl back.

Completed walk around with Consultant Billy Wells revealing proper secondary containment was in place and the fuel skid providing fuel for the frac job was in operation and supplied a total of 83720cuft of natural gas for the stage.

1499693024 2017-07-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a production tree and a lubricator for future use and valves were in closed position. The production equipment included a sand trap; a line heater, 2-GP units and 2-400 barrel purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Annular pressure monitoring was present along with proper identification and production records being current. An Above Ground Storage Tank (AST) was present on the northwest corner of the well pad supplying fresh water to the Frank Miller pad for frac with support being provided by Great Lakes. Water was field tested revealing chlorides of <33 parts per million, pH 7 and 60 degrees.
1528730041 2018-06-11 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. A gauge was present on the production tubing reading 580 pounds per square inch, 860 pounds per square inch on the production casing, 600 pounds per square inch on the 9.625 inch diameter intermediate casing, and 150 pounds per square inch on the 13.375 inch diameter surface casing. The production equipment consisted of two 400 barrel tanks, six sand traps, six gas production units, and one indirect line heater. An above ground storage tank was situated in the Northwest corner of the pad. The identification was located at the wellhead. The area effected by the pad construction does not have any erosion present and vegetation was established. The pad surface consisted of gravel with a fence surrounding the perimeter. The location meets preliminary restoration requirements written under 1509.072 (A).
