0018791898 2013-10-17 Plug / Plug Back No Violations The cement top was found at 18. We topped off the well with left over cement from setting a cement plug on the previous well.
1052432486 2013-10-15 Plug / Plug Back No Violations The 4.5 casing was attempted to be pulled several times but the telegraph was lost each time it was attempted. Free pipe was found around 800 feet but it could only be shot off around 510. The casing was mudded in so it was pulled and 2 3/8 tubing run into the hole. The well was cemented to surface after the hole was heavily conditioned to remove the clay used to mud the casing in. Cement was brought to surface but it will likely need to be topped off do to the hole taking the cement plug.
1382317785 2013-10-16 Plug / Plug Back No Violations We were setting the final top plug for this well. Cement was tagged at 757 after a 45 sack plug. We tried to rip the 4.5 casing several times but were forced to shoot it off at 510. We ran a 145 sack cement plug and circulated it to surface. The well had to be topped off several times as the cement was falling. The cement top will be checked the following day.
1426719092 2015-03-18 Final Restoration No Violations On the date of inspection I am accompanied by SE Regional Manager Brent Bear to inspect the plugged and abandoned well for Final Restoration. The well passes Final Restoration per ORC 1509.072(B).
2119656804 2013-10-04 Plug / Plug Back No Violations New GPS coordinates were taken for this well to determine if the proper API# has been assigned to the well. The current well does not appear to be of the age stated on the well card. After mapping the well the well is offset but does appear to be the correct numbering. This well will be soon plugged.
